People suffering from pancreatic tumors are eager to receive aggressive and effective treatment. But what is the ideal “positive” and “effective” treatment? Due to superficial knowledge and fear of pancreatic tumor and blind faith in medical treatment, the main problem is not “not active” but “too active”. “When the diagnosis is still to be clarified, various invasive and irreversible treatments are launched at once. Objectively speaking, the value of existing medical treatments for most pancreatic cancers is limited, especially expecting that most of them will be disappointed by the current technology to cure pancreatic cancer radically. On the contrary, it may be more realistic to focus more on improving symptoms and delaying life. Effective treatment: The following are the main indicators to determine whether the treatment is effective. 1.Change of tumor size: Only for measurable mass lesions, not for diffuse lesions and small lesions which are difficult to measure. 2.Change of laboratory index: mainly refers to CA19-9, if this index is normal or only generalized >1000 without specific value, it has no judgment value. During the treatment, the decrease of CA19-9 should exclude the interference of improvement of biliopancreatic inflammation in addition to the consideration of effective treatment. 3. Changes in subjective symptoms: abdominal pain is the late manifestation of most pancreatic cancer patients, with different manifestations and great differences in degree, and it is most affected by psychological effects, so it has limited reference value to be an index for efficacy assessment. Objectively speaking, all the above three indicators have ambiguous space and limited judgment value. Conversely, if there are no clear indicators of the above three, is it pancreatic cancer? Whether it needs to be treated too aggressively or not are debatable issues.
