Does early stage lung cancer still need treatment after surgery?

Some patients with early-stage lung adenocarcinoma require follow-up adjuvant therapy after surgery, while others do not require treatment and can be observed. If the pathology after surgery indicates that the cancer is in situ or stage I, that is, the tumor is not large, does not invade important organs, and there are no lymph node metastases, in this case, both Chinese and foreign guidelines recommend that adjuvant chemotherapy is not needed. If the postoperative pathology shows that the tumor is not completely excised, the margins are still positive or the tumor is also early but has reached stage II, most often the tumor is not large, maybe only 2-3 cm, but there is a lymph node metastasis in the hilum, even if there is only one lymph node, it is still considered relatively early, such as stage IIA, but postoperative adjuvant chemotherapy is still recommended. If the surgical margins are positive, it means that the lymph nodes are not completely cut, and in this case, radiation therapy may be required.
