I have been taking Flutamide after my orchiectomy and now, I find that my PSA is elevated to the level it was before the surgery. My doctor told me to stop taking Flutamide, but I am scared, can I stop? Will it be effective if I switch to other medications?  If the PSA rises to the pre-surgical level after endocrine therapy, it may develop into an early state of hormone-independent prostate cancer, where endocrine therapy is ineffective. At this time, the drug taken not only fails to block the function of the androgen receptor, but may even become an agonist of the androgen receptor, which means that the drug may have changed from a therapeutic effect to a tumor-promoting effect. This is called “anti-androgen withdrawal syndrome”. At this point, if the PSA rises again after stopping the drug, the disease may be controlled again by switching to other anti-androgen drugs, which is called second-line endocrine therapy. Generally speaking, when the disease has progressed to this point, it is not far from the stage of hormone non-dependence.
