Experience in treating chronic cough from the spleen and stomach theory

  Cough is one of the most common clinical symptoms. Most patients with cough can be cured with appropriate treatment, but there are some patients who have cough symptoms that persist for a long time, causing a lot of trouble and financial stress to the patient. In addition to some patients with a clear diagnosis, there are also some patients who cannot be diagnosed by clinical examination, making treatment quite difficult. In clinical practice, I often encounter patients with coughs that do not go away after being treated by Western medicine. For such patients, the author has achieved better results with the application of Chinese medicine. It can be said that Chinese medicine is an effective means of treating prolonged cough, which not only has a flexible theoretical basis of diagnosis and treatment, but also has effective prescriptions under the guidance of Chinese medical theory.  As a Chinese medical proverb says, “All the five internal organs and six internal organs can cause cough, not only the lungs”. One of the basic principles of TCM treatment is the holistic view. The five viscera and six internal organs of the human body are a whole and not only support each other physiologically and coordinate with each other, but also affect each other pathologically. Although cough is a symptom of the lungs, its development is closely related to other internal organs. Only a profound understanding of the pathophysiological relationships between the internal organs can enable us to know the source of the disease clinically and to effectively formulate and use drugs. This is the basis for achieving good results. Among the internal organs of the human body that are related to cough, the author believes that the main organs are the lung, spleen, stomach and liver. The spleen and stomach play an important role in the pathogenesis and treatment of chronic cough.  The theory of TCM originates from ancient philosophy. According to ancient philosophy, before heaven and earth were divided, there was a chaos of qi. Qi contains yin and yang, with the clear being yang and the cloudy being yin. Between yin and yang is the middle qi. The middle qi is the hub of yin and yang. The pivot rotates, the clear yang from the left and up, it is wood, wood is rising, its temperature; clear yang up in the upper, the accumulation of heat is fire, fire is hot, its gas is hot; turbid yin from the right and down, it is gold, gold is astringent, its gas is cool; turbid yin all down in the lower, the accumulation of cold into cold, it is water, watery hibernation, its gas is cold. Wood, fire, gold, water, that is, the rise and fall of the four changes in the Qi. The earth lives in the center, in the four seasons each 18 days, its gas wet. Therefore, the earth is called the five elements by combining the four signs. The human body is also a small world, with the virtues of the five elements within. The five elements of the human body exist in a relationship of mutual production and mutual conflict, together maintaining the balance of the human body. Wood is dispersed, gold grams wood is not too dispersed; fire is inflammatory, water grams fire is not too inflammatory; earth is wet, wood sparse earth, the earth is not too wet; gold astringent, warm with fire is not too astringent; water descends to moisten, seepage of the earth is not too moist. The earth is in the center, is the pivot of the qi up and down. The leftward rotation of the central qi is transformed into hexa-earth, which is in the spleen, and the rightward rotation of the central qi is transformed into w-earth, which is in the stomach. The spleen is responsible for raising the clear yang, while the stomach is responsible for lowering the dark yin. When the spleen earth rotates to the left, the yin rises and transforms into the clear yang, which is the liver and gallbladder. The clear yang rises above the heart and small intestine. When the stomach is turned to the right, the yang descends and transforms into turbid yin, then the lungs and large intestine. The cloudy yin descends to the kidney and bladder. Therefore, the five viscera and six bowels are all created by the rising and falling of the middle qi. The middle qi is the hub of water and fire, the central axis of lifting and lowering gold and wood, and the source of transformation of the five viscera and six bowels. The spleen and stomach are the hub of the body’s qi flow. The spleen prefers dryness to dampness, and is the source of blood production; the stomach prefers moistening to dryness, and is the source of qi transformation. The spleen rises to clear the yang, the spleen rises to the liver and kidneys; the stomach is the main descending turbidity, the stomach descends to the heart and lungs and descends. In this way, the water and wood are not depressed, fire and gold are not stagnant, the upper clear and lower warm and the body is healthy and healthy. The spleen is wet with wet earth and wet nature, the stomach from dry gold and dry nature, both dry and wet is not biased, then the vitality of the middle, so the essence of water and grain can be transformed into qi and blood nourishing the five organs, the essence of the spirit of prosperity, all diseases are not born. If there is an imbalance of dryness and dampness due to external and internal injuries, the middle qi decays and the spleen and stomach lose their elevation. If the clear Yang of the spleen does not rise, transportation and transformation will be delayed; if the clear Yang of the spleen sinks, the spleen family will be deficient in cold. If the Spleen and Earth do not rise, the formation of Zong Qi is affected. Insufficient zong qi causes the lungs to lose their ability to promote the essence of water and grain, resulting in the internal growth of phlegm and stagnation of the lungs. Failure to raise the Spleen Earth can also lead to Liver and Kidney stagnation. Stagnation of liver wood will lose its function of draining; stagnation of kidney water will lead to lower cold and loss of warmth. If the kidney water is stagnant, the fire in the kidney can’t warm the spleen and stomach, resulting in the failure of the central axis of the spleen and stomach and the lack of middle energy. If the cloudy yin of the stomach does not descend, there is no right to receive and receive; if the cloudy yin of the stomach goes up, the heart and lungs do not descend. If the lung is stagnant, it loses its purifying function and coughs. The liver and gallbladder are wood, and the temperature of wood is warm, so it is the master of ascension and development. Liver wood is born in kidney water and grows in spleen earth. If water and earth are warm, liver wood rises and develops smoothly. The liver is the master of life and its qi is wind. If the water and earth are not warm, the liver will be injured and depressed, and the liver wind will be generated internally, then all the symptoms will be made. The bile wood is transformed into qi in the phase fire, the liver wood is depressed, then the wind moves to line its drainage, impulse the phase fire, bile wood with the phase fire to reverse the rise and sentence the lung gold.  Therefore, although cough is a lung disease, it is closely related to the spleen, stomach, liver and kidney. The spleen and stomach are responsible for the rise and fall of the qi and play an important role in the pathogenesis of cough, which is the theoretical basis for the treatment of prolonged cough from the spleen and stomach. In patients with prolonged cough, the author first considers whether there is a deficiency of the spleen and stomach. In the case of Spleen-Qi deficiency, the coughing sound is low, the coughing sound is sometimes white, the tongue is light red with white or greasy coating, and the right guan pulse is thin and sunken or weak, so the formula is based on Si Jun Zi Tang with addition and reduction; in the case of Stomach-Yin deficiency, the coughing sound is not smooth, the tongue is red with white coating and dry or yellow, and the right guan pulse is thin and counted, so the formula is based on Yiguan Tang with addition and reduction. Secondly, the presence or absence of fire in the liver and gall bladder should be considered. If the cough is strong, the chest is stuffy and suffocating or the chest and ribs are painful, and the left guan pulse is thin or slippery, the evidence belongs to liver-yin deficiency and wood fire upflammation, then use Ume plum, white peony, lady’s mantle and wolfberry to nourish its yin, scutellaria, hooked vine, Xia Ku Cao and Chai Hu to lower its fire, and niu knee to guide the downward flow of qi. If the left guan pulse is weak or stringy, although there is no liver fire, we should also consider the evidence from liver qi deficiency and add wolfberry, dogwood, white peony, sour date, etc. to warm liver qi and help its draining power. The third defense of the presence of deficient kidney qi. If the right ruler is sunken and weak, the fire of the vital gate is weakened, add or subtract Right Return Pill; if the left ruler is sunken and weak or sunken, add or subtract Left Return Pill. In conclusion, the treatment of prolonged coughing can be achieved by dialectical prescription from the spleen and stomach.  Case example: Wang, a male, had a cough for more than 3 months. His cough is frequent, his sputum is white and small, his throat is dry with a feeling of sticky phlegm stagnating in the throat, his left inch is slippery, his left guan is thin and his right guan pulse is thin and slippery, and his tongue is red with white coating and dry. Herbs: 15g of Yam, 18g of Lentil, 18g of Lotus, 10g of Radix Scutellariae Sinensis, 9g of Scutellariae Sinensis, 12g of Radix Scutellariae Sinensis, 9g of Ocimum sanctum, 15g of Tribulus terrestris, 15g of Tribulus terrestris, 12g of Roasted Loquat leaf, 10g of Paeonia lactiflora, 9g of Radix Platycodon, 12g of Zhi Mu, 9g of Burdock. 7 doses, the cough decreased, and the symptoms improved significantly with 7 more doses.  Analysis: Although the patient had cough as the main symptom, the tongue and pulse showed that the patient had both the basis of stomach yin deficiency and liver fire upflammation. Therefore, we nourished stomach yin with yam, lentils, lotus seeds and northern sage to restore the lift of the central axis; scutellaria, tong tribulus terrestris, white tribulus, lady’s mantle, wu mei, white peony and roasted loquat leaf to nourish liver yin and clear liver fire so that wood qi is not depressed. The only treatment is to astringent the lung with Radix Platycodon, Zhi Mu and Burdock to stop cough and resolve phlegm. Although it does not focus on stopping cough, it receives better results because it takes a holistic approach and coordinates the relationship between the lung, stomach and liver. In conclusion, although the disease of prolonged cough is small, the pathogenesis is complex, so we need to start from the whole picture, combine the four diagnoses and use one or several methods to understand the pathogenesis and use the right medicine to achieve good results.
