Pancreatic cancer is a common clinical malignancy of the gastrointestinal tract, and its incidence has been on the rise in recent years. In the United States, pancreatic cancer is the 4th most common tumor causing death, and in China, its incidence has increased about 6 times in the past 20 years. It is difficult to detect pancreatic cancer at an early stage, and once detected, 80% of patients lose the chance of surgery. Even in patients with limited lesions, despite radical surgery, recurrence and metastasis still occur, with a 5-year survival rate of less than 25.0%. The 1-year survival rate of patients with advanced pancreatic cancer is less than 10%, and the median survival time is only about 3 months. After 5-FU treatment, the median survival time is only 5 months. Chinese medicine has its unique features in the treatment of pancreatic cancer, and its efficacy is quite satisfactory. The recent research on Chinese medicine for pancreatic cancer in the past 10 years is summarized as follows: 1. According to Wang Qingcai [4], the disease is mostly caused by depression of the seven emotions or dietary disorders, which leads to damage of the liver and spleen, loss of harmony of the internal organs, obstruction of spleen movement, internal accumulation of damp-heat and internal stasis of toxins. Qiu Jiaxin [5] believed that spleen deficiency is the root of the disease in patients with pancreatic cancer, although sometimes there are manifestations such as toxic heat, dampness obstruction, phlegm coagulation, qi stagnation and blood stasis, all of which are derived from spleen deficiency. Pancreatic cancer is due to three causes: first, damp-heat in the spleen and stomach: it is caused by poor qi flow, spleen-dampness and depression, which turns into heat over time, and damp-heat accumulates and becomes toxic over time; second, stasis in the liver and spleen: it is caused by depression and anger, liver-depression and stagnation of qi, poor diet, excessive consumption of thick food, loss of spleen transportation and transformation, and pain in the chest and diaphragm; third, solid heat in the heart and spleen: it is caused by the presence of toxic heat, depletion of yin and blood, internal heat due to yin deficiency, delusion of blood-heat, and inflammation of heart fire. According to Zhao Jingfang [7], dysfunction of the spleen and stomach in the middle jiao is the key to this disease. Only on the basis of regulating the acquired spleen and stomach with the method of regulating qi, resolving dampness, and eliminating accumulation can the medicine hit the mark. 2.Treatment of pancreatic cancer based on the principle of strengthening the spleen and regulating qi, with the addition of drugs for clearing heat and detoxifying, dispelling dampness and phlegm, softening hardness, moving qi and activating blood. The basic formula is composed of prunus ginseng, fried atractylodes, poria, hen’s nest, red vine, Xia Ku Cao, oyster and sarsaparilla, etc. It was used to treat 16 cases of pancreatic cancer. As a result, 4 patients who failed to operate survived from 6 to 15 months, 9 cases who underwent palliative rerouting survived from 9 to 32 months, 3 cases who underwent radical surgery, 1 case has survived 57 months, 2 cases have survived more than 5 years, and the longest has now survived 74 months. (1) Damp-heat and poisonous type with Huanglian Detoxification Tang and Yin Chen Artemisia Tang plus or minus; (2) Stagnation of Qi and stagnation type with Curcuma San plus or minus; (3) Spleen deficiency and damp-heat type with Xiang Sha Liu Jun Zi Tang and Exhaustion Drink plus or minus; (4) Positive deficiency and evil solid type with Ginseng and Mai San and Sha Shen Mai Dong Tang plus or minus. In the early post-operative stage, patients with advanced pancreatic cancer undergo internal bile and intestinal drainage, and adopt the method of clearing heat and detoxification, draining the liver and bile, activating blood circulation and removing blood stasis, and promoting internal attack. In the late postoperative period, the formula was based on the support of the root of the disease, and the formula was based on the use of ten whole tonic soup, ascending pulse san, and tonic zhong yi qi tang. In order to prevent pancreatic fistula, nutritional support of santodine was given, and conventional chemotherapy drugs 5-FU and furanyl fluorouracil were used postoperatively. As a result, 15 cases had no postoperative complications, and only 2 cases died after discharge, surviving 11 and 12 months; the remaining 13 cases had survived for an average of 11.1 months. Ultrasound-guided local injection of drugs (cisplatin 10-20mg, diluted with 10-20ml of pure ethanol), percutaneous transhepatic biliary support tube for bile drainage and the treatment with Chinese medicine “Pancreatic Bao Kang Tai” achieved satisfactory results. Pancreatic Treasure Kangtai is composed of raw Job’s Tear, Dongling Cao, White Flowering Serpent’s Tongue and Tongue, Buddha’s Aegis, Swollen Wind, Atractylodes Macrocephalae, Trigonellae, Curcuma longa, Astragalus and Chuanxiong. The results showed that the six-month, one-year and two-year survival rates were 88.7%, 68.6% and 51.4%, respectively, with a median survival period of 10.3 months. The patients were divided into three groups: Chinese medicine group, chemotherapy group and interventional group. In the Chinese medicine group, the formula was added and subtracted with Diaphragmatic Expulsion of Blood Stasis Tang. The chemotherapy group was treated with Diaphragmatic and Yu-hua Tang plus reduction + short course of systemic chemotherapy. In the interventional group, the formula was added and reduced with Diaphragmatic and Yuyu Tang + arterial intubation chemotherapy perfusion. Clinical observation showed that the arterial cannulation chemotherapy with Chinese medicine group had the best effect with an effective rate of 23%; the six-month, one-year survival rates were 83% and 58.3%, respectively. Li Qiu [16] randomly divided 82 patients with inoperable advanced hepatocellular carcinoma and pancreatic cancer into three groups and gave them Ginseng-Qi cancer suppressing liquid, cytokines and conventional chemotherapy drugs respectively. Ginseng and Astragalus cancer suppressing solution was purified from ginseng, Astragalus, Toadstool and Zanthoxylum by injection. The results showed that the survival time of patients in Ginseng-Qi cancer suppressing liquid group, cytokine group and conventional chemotherapy drug group were 11.3 months, 9.5 months and 8.5 months, respectively; Ginseng-Qi cancer suppressing liquid group and cytokine group were better than conventional chemotherapy drug group in improving symptoms. 4.Experimental study of Chinese medicine for pancreatic cancer Arsenic trioxide is the main active ingredient of traditional Chinese medicine arsenic and Xiong Huang, which have a long history of use in Chinese medicine. The pancreatic cancer cell line SW1990 was treated with different concentrations of arsenic trioxide, and cell proliferation was measured by MTT method, and apoptosis was detected by electron microscopy, flow cytometry and immunohistochemical staining. The results suggested that arsenic trioxide could effectively inhibit the growth of SW1990 cultured in vitro; induce cell cycle arrest and apoptosis in SW1990 cell line; and significantly inhibit the growth of subperitoneal pancreatic cancer transplantation tumor in nude mice. Different concentrations of arsenic trioxide were co-incubated with pancreatic cancer cell lines SW1990 and SW8902. The formation of apoptotic vesicles was observed by HE staining light microscopy, subdiploid apoptotic peaks appeared before G1 phase by flow cytometry, and apoptotic characteristic Ladder was found after cell DNA extraction electrophoresis, again suggesting that As2O3 can induce apoptosis in pancreatic cancer cells. Hydroxycamptothecin is an extract of the Chinese herbal medicine Camptothecin. The in vitro cultured human pancreatic cancer cell line SW1990 was treated with different concentrations of hydroxycamptothecin and cell proliferation was measured by MTT assay, and apoptosis was detected by Annexin V early apoptosis detection kit, electron microscopy, flow cytometry and in situ end-labeling analysis and bcl-2 immunocytochemical labeling analysis, the results suggested that hydroxycamptothecin could inhibit SW1990 cell proliferation in vitro, and part of the mechanism of action is induction of apoptosis, which can be used as an apoptosis inducer for pancreatic cancer treatment. Xu Rutang [22] reported that Japanese researchers used human pancreatic cancer cell line SUTT-2 cells for in vitro monolayer culture with the addition of 5-FU and the Chinese herbal medicine Chai Hu Gui Zhi Tang, etc. The results showed that Chai Hu Gui Zhi Tang could inhibit the repopulation of human pancreatic cancer SUTT-2 cells and was superior to the effect of 5-FU alone. 5. In conclusion, most traditional Chinese medicine practitioners believe that pancreatic cancer is caused by dysfunction of the spleen and stomach and the interplay of dampness, heat, stasis and toxicity. Most of the treatment is based on the principle of strengthening the spleen and regulating the qi, with the addition of heat-clearing and dampness-relieving herbs. Thus, we can see that Chinese medicine has its advantages in the treatment of pancreatic cancer, especially the combination of Chinese medicine with radiotherapy and interventional chemotherapy can reduce toxicity and increase effectiveness and improve clinical efficacy. However, there are too few clinical control studies of TCM in pancreatic cancer, mostly retrospective clinical reports, and there is a lack of prospective, large-sample, multicenter clinical studies, and there are even fewer experimental reports on the mechanism of TCM treatment of pancreatic cancer. Clinical and experimental studies on the combination of Chinese and Western medicine in the treatment of pancreatic cancer need to be further developed. This will be an important way to improve the clinical treatment effect of pancreatic cancer.
