Hypoechoic category 4a nodule in the left lobe of the thyroid gland is a suspected malignant nodule, which requires further examination to clarify the nature of the nodule, and regular follow-up or surgical treatment, which should be performed according to the doctor’s instructions. 1. Hypoechoic category 4a nodule in the left lobe is the result of thyroid ultrasonography and is reported according to the TI-RADS grading method. According to the TI-RADS classification, thyroid nodules are categorized into 6 categories. category 1 is negative, category 2 is definitively benign, category 3 is probably benign, category 4 is suspected malignant, category 5 is definitively malignant, and category 6 is biopsy-proven malignant. 2. Thyroid category 4 nodules are subdivided into 4a, 4b and 4c. Category 4a nodules represent patients with a 2%-10% likelihood of malignant thyroid nodules; category 4b nodules have a 10%-50% likelihood of malignancy, and category 4c nodules have a 50%-90% likelihood of malignancy. However, the benign or malignant nature of the tumor needs to be further clarified by puncture biopsy. 3. For category 4a thyroid nodules, further fine-needle aspiration of the thyroid gland is recommended to clarify the diagnosis. If the test result is benign, regular and close review is recommended; if the test result is malignant, surgery for radical treatment of thyroid nodules is required. If patients are found to have thyroid nodules of category 4a, it is recommended that they consult a doctor as soon as possible to clarify the nature of the nodules so as to avoid delays in their condition.
