Abstract: An increasing number of patients with cardiovascular and cerebrovascular disease have a combination of psychosomatic problems. These two types of diseases interact with each other and together lead to the deterioration of the disease. Based on the “double heart medicine” model, we propose the “mind-body treatment” therapy to prevent and treat cardio-cerebral vascular diseases, which means that while giving physical medication to patients, we pay attention to their psychosomatic treatment. The Chinese medicine idea of “treating the disease before it happens” and the “seven emotions regulating health” and “food therapy and medicinal food for health” are carried out in the daily life of patients. It provides a new way of thinking for combining Chinese and Western medicine to prevent and treat cardiovascular and cerebrovascular diseases. Cardiovascular disease has become the leading cause of death in the adult population in China, with nearly 3 million people dying from cardiovascular disease each year. Due to the prevalence of cardiovascular disease risk factors and unhealthy lifestyles, the incidence of cardiovascular and cerebrovascular diseases in our population is on a rapid rise. These diseases are long-lasting, widespread, expensive, and have a high rate of disability and death. According to WHO, 80% of cardiovascular and cerebrovascular diseases are related to lifestyle, and the most harmful risk factors are unhealthy diet, inactive lifestyle and smoking. An unhealthy diet and inactive lifestyle can lead to increased blood pressure and blood sugar, dyslipidemia, and overweight and obesity, which in turn increase the risk of cardiovascular disease. In addition, psychological and social stress and emotional agitation are also important risk factors for cardiovascular disease that should not be ignored, and these are the current hot spots of concern in the field of cardiovascular disease prevention and treatment. 1.Psycho-psychological problems and treatment status of patients with cardiovascular and cerebrovascular diseases With the development and progress of society, the medical model has changed from the pure biomedical model in the past to the biopsychosocial medical model, which requires the majority of medical personnel to understand and deal with diseases from the viewpoint of the biopsychosocial medical model. In our clinical practice, we found that more and more patients with cardiovascular and cerebrovascular diseases are combined with psychosocial problems. Psychosocial problems mainly originate from three aspects: ① Psychiatric disorders secondary to cardiovascular diseases: Patients suffering from organic heart disease or stroke experience emergency and surgery, plus a variety of maladjustments after the disease, lack of understanding of the prognosis of the disease, unawareness, leading to psychological trauma, resulting in anxiety, depression and other psychological problems. (2) Mental disorder-related “symptoms of heart and brain disease”: For example, patients with “mental disorder” who present with “chest tightness, palpitations, dizziness and other symptoms” are very common in outpatient clinics, and their underlying causes The underlying cause may be depression, anxiety, or somatization disorder. Patients present with physical discomfort or somatic symptoms without evidence of corresponding physical illness (i.e., positive signs and positive test results). (3) Medical “emotional problems”: The medical staff did not objectively and impartially evaluate the disease and prognosis, and intentionally or unintentionally increased the patient’s mental burden. Or because of the lack of communication and communication skills with patients, resulting in unnecessary misunderstandings. It is all these reasons that cause the increasing co-morbidity of cardiovascular diseases with depression and anxiety disorders. Some research data show that patients with cardiovascular disease have high rates of concomitant depression and anxiety, 22.8% and 70.9%, respectively, and the incidence is higher in women than in men; the incidence of depressive disorders after stroke is 36%, and mild depression accounts for 80% [6]. Psychosomatic factors are risk factors that promote the development of cardiovascular and cerebrovascular diseases, and these two types of diseases interact with each other and together lead to the deterioration of the disease. Due to the guidance of the traditional pure biomedical model, the current treatment model in the vast majority of hospitals is disease-centered rather than patient-centered, and large hospitals emphasize overly detailed and specialized subspecialties, leading to the division of the totality of medicine, resulting in a large number of patients with psychological disorders being scattered in various departments due to somatic symptoms, while doctors treat the headache and the foot, focusing only on solving somatic diseases and failing to identify and They cannot identify and effectively treat patients’ psychosomatic problems in time, or even be misdiagnosed or mistreated. This may be one of the reasons for the high incidence rate, high disability rate, high death rate, high recurrence rate and many complications of cardiovascular and cerebrovascular diseases in China, which are “four highs and one more”. 2. Clinical application of “mind-body treatment” and its efficacy The ultimate goal of medical development is to love and care for people. As doctors, we must first have a benevolent heart, and then a benevolent art. In view of the current situation that patients with cardiovascular and cerebrovascular diseases have a wide range of mental and psychological problems, according to the “two-hearted medicine” model, we start from clarifying the whole picture of the disease and caring for the sick person, and transform the concept of “treating people with medicine” into “treating people with heart”. “We propose the “mind-body treatment” therapy to prevent and treat cardio-cerebral vascular diseases, that is, while giving patients physical drug treatment, we also pay attention to their mental and psychological treatment. Patients with cardiovascular and cerebrovascular diseases, whether it is the onset or the progress of the disease, are closely related to the patient’s lifestyle, and most of these patients are chronically and seriously ill and need long-term treatment. For patients and family members who see a doctor, the first thing they seek is a psychological treatment. However, in China, psychological treatment is still very uncommon, and there are many people whose mental illnesses accumulate over time and finally manifest themselves through physical illnesses. Therefore, we need to repair not only the physical body but also the psychological order. Only with the trust and cooperation of patients can we achieve good treatment results. The specific practices of “mind-body treatment” include opening special outpatient clinics for cardiovascular and cerebrovascular diseases, establishing patient files according to the type of disease, choosing “mind-body treatment” to optimize the treatment plan, and supervising patients to standardize and reasonably use medication; guiding them to correct their poor lifestyles (including poor We also provide patients with psychological problems with psychological guidance and various behavioral interventions; regularly conduct cardiovascular and cerebrovascular health education lectures within the hospital or departments to disseminate knowledge of cardiovascular and cerebrovascular health and popularize the general knowledge of TCM health care; and put the TCM “treating the disease” idea into practice. The idea of “treating the disease before it happens” and the “seven emotions regulating health care” and “food therapy and medicine health care” are carried out in the daily life of patients. After nearly three years of practice, we have achieved very good results and are welcomed by the majority of patients. Under the specific guidance of medical and nursing staff, patients’ medical compliance has improved significantly, and they are able to consciously and more rationally use drugs in a standardized manner. The ability of self-care and health care has been enhanced, their moods have improved, their mental conditions have significantly improved, and the majority of patients have stabilized or fully recovered from their illness, greatly improving the quality of survival. Since December 2008, our group has participated in the clinical validation of the treatment plan for hyperlipidemia, the main disease of the 11th Five-Year Plan of the State Administration of Traditional Chinese Medicine, and has provided free lipid testing, consultation, health education, behavioral guidance and medication guidance for more than 1300 Guangzhou citizens. We have conducted a randomized double-blind clinical study of a certain scale to provide a basis for the effectiveness of Chinese medicine in treating hyperlipidemia. In order to better popularize health knowledge, do a good job in primary and secondary prevention of cardiovascular and cerebrovascular diseases, reduce the morbidity and mortality of cardiovascular and cerebrovascular diseases, and reduce the social and economic burden caused by cardiovascular and cerebrovascular diseases, we will actively promote the “mind-body-treatment” therapy. “We intend to go out of the hospital and into the community, take the street community as the basis, realize the integration of hospital and community, improve the ability of community doctors to identify and treat cardiovascular diseases, increase the content of community medical services, and promote the improvement of community diagnosis and treatment. At the same time, we will further expand and provide communication channels for doctors and patients, and create the “Cardio-Cerebral Vascular Disease Mind-Body Treatment Forum” as a platform for doctors and patients to communicate, so that the new medical model of bio-psycho-social medicine can be better implemented and implemented, and the prevention and treatment ability of cardio-cerebral vascular diseases can be improved. In addition, besides serving patients with cardiovascular and cerebrovascular diseases, we will also pay attention to people in sub-healthy state, which is a major proposition of human medicine in the new century. According to the statistics, 70% of people in China are in sub-healthy state, and sub-healthy state has become a common social problem. Chinese medicine has rich theories and methods in diagnosing, preventing and treating the subhealthy state through the idea of “treating the disease before it happens”, Chinese medicine somatology, psychosomatic medicine and non-pharmaceutical therapy. We intend to educate subhealthy people through health education, health examination, medical consultation, psychological counseling, community medical treatment and other forms of scientific propaganda on the prevention and health care of cardiovascular and cerebrovascular diseases, enhance the health awareness of this group, make them realize and understand the influence of psychological, social and environmental factors on health, actively control the risk factors of cardiovascular and cerebrovascular diseases, and have far-reaching significance in reducing the incidence of cardiovascular and cerebrovascular diseases. far-reaching practical significance.
