Do all chocolate cysts come back after surgery?

There is a possibility, but not absolute, of recurrence after chocolate cyst surgery. Chocolate cysts are cysts encasing fluid accumulation formed by endometriosis into the ovaries and repeated bleeding with the menstrual cycle, usually leading to menstrual cramps and infertility in patients. Chocolate cysts can be handled surgically. After surgery, some of the endometriotic tissue may still remain and may re-form as a blood-accumulating lesion, leading to recurrence of the chocolate cyst. Therefore, after chocolate cyst surgery, leucovorin is usually used for about 3 months as prescribed by the doctor to prevent recurrence after surgery.
