Patient: Description of condition (onset, main symptoms, hospital visited, etc.): Patient male, 17 years old, has had nail infection for three years, nail cut on one side in hospital in 2009, nail cut on both sides in Beijing Children’s Hospital last month, usually bleeding, no more bleeding after removing part of the toenail, but new toenail grows out and still grows into the flesh. Can the surgery be curative, do I need to be hospitalized, how long do I need to rest, how do I get anesthesia during surgery, and does it hurt? A: Cutting or pulling the nail alone is a simple surgery, but it can only drain the nail. Although the inflammation can be controlled quickly for the time being, once the nail grows out, the nail will still be embedded in the nail contour tissue, which will lead to recurrence. Therefore, the contradictory relationship between the nail body and the nail contour must be resolved. The nail body is narrowed and the nail contour is widened. We are now using nail extraction + partial excision of the nail bed + nail contouring. This can treat the ingrown nail more thoroughly and reduce the recurrence rate. The surgery is performed under local anesthesia, which is not painful (but is painful when the anesthetic is injected) and does not require hospitalization, but the medication needs to be changed after the surgery. The total healing period is about 2 to 3 weeks.
