Prevention of allergic rhinitis combined with asthma

Bronchial asthma and allergic rhinitis are common and frequent clinical diseases, and their incidence is increasing year by year. Epidemiological surveys have confirmed that the incidence of asthma is 4-20 times higher in patients with allergic rhinitis than in normal people, and it is even believed that 60% of allergic rhinitis may develop into asthma or be accompanied by lower respiratory symptoms. The development of allergic rhinitis into asthma is closely related to the amount and concentration of allergen exposure. Clinically, patients often feel short of breath and tightness in the chest, and the symptoms are aggravated at night, with phlegm in the throat, and in severe cases, they cannot lie down at night and need to breathe. Its attacks are often related to the change of seasons, uneven hot and cold climate, improper diet, fatigue and stimulation of odor. Autumn and winter is one of the most frequent seasons, which not only brings unimaginable pain to the patients, but more importantly, the serious complications often brought by the long-standing “wheezing”. At present, western medical treatment mainly focuses on the combined anti-inflammatory treatment of upper and lower respiratory tract inflammation in allergic rhinitis and asthma, with inhaled glucocorticoids as the mainstay, which can help stop the occurrence of asthma or reduce the severity of lower respiratory tract symptoms, but many patients do not know enough about the disease and pay enough attention to it, and are afraid of the side effects of hormones and other western drugs, so the use is not satisfactory, making the current status of the treatment of this disease not optimistic. Chinese medicine treatment emphasizes a holistic concept, targeting “one airway, one disease” to carry out overall conditioning and treatment, and at the same time, according to the different physical conditions of patients, especially after the stabilization of the disease, the use of Chinese medicine prescriptions, customized, targeted, to support the deficiency, to prevent recurrence and aggravation of the disease. The Chinese herbal medicine prescriptions have been welcomed by more and more patients. Case 1: Patient Yuan Moumou, female, 45 years old, complained of recurrent coughing, wheezing, phlegm and tinnitus for 3 years, aggravated for nearly a month. The patient has suffered from allergic rhinitis for many years, often due to changes in the environment, climate and other changes in nasal sneezing, often with nasal congestion. In the past 3 years, he often feels short of breath at night and occasionally has phlegm in his throat, which is triggered by climate change, and was temporarily relieved by taking Montelukast tablets, but his symptoms remained after stopping the medication. In the past 1 month, he often feels tightness and shortness of breath in the chest at night, and wheezing when he moves, or he has phlegm in the throat, and in the morning he coughs with white color but not yellow, sweats spontaneously, is afraid of wind, coughs easily in the wind and cold, is tired, has a light mouth and is not thirsty, is afraid of cold, often feels weak in the back and legs, has clear nasal mucus, sneezes more, has a small amount of food, and has a regulated stool. The tongue is light red with thin coating, and the pulse is weak. Physical examination: no obvious croup in both lungs. Pulmonary function tests suggest mild obstructive ventilatory dysfunction and bronchodilator test (+). Traditional Chinese medicine diagnosis: croup; Western medicine diagnosis: allergic rhinitis combined with asthma. Comment: The diagnosis of TCM is deficiency of lung, spleen and kidney, failure of the lung guard, failure of spleen movement and phlegm-drinking. The treatment is to strengthen the spleen and tonify the lung, benefit the qi and tonify the kidney, warm the yang and dissolve the drinks, and regulate with cream instead of decoction. The formula selects Yu Ping Feng San, Tonic Zhong Yi Qi Tang plus or minus (Astragalus, Atractylodes, Radix Codonopsis, Radix Angelicae Sinensis, Radix Bupleurum, etc.) to nourish lung qi, Jing Fang Defeat Poison San plus or minus (Thornbush, Fang Feng, Qiang Wu, Chuanxiong, Su Ye, Cang Er Cao, Xin Yi, etc.) to promote dispersion and open the orifices; Six Jun Zi Tang (Prince’s Ginseng, Atractylodes, Semen, Chen Pi, Yam, Coix Coix Seed, Poria, Chicken Nei Jin, Sha Ren, etc.) to strengthen the spleen and resolve phlegm; based on the fact that drinking is a yin evil, it is warmed to open, and Yang to transport. Based on the theory that drink is a yin evil, it is warmed to open and yang to transport, the formula uses Xiao Qing Long Tang plus reduction (Ephedra, Chuan Gui Zhi, Hsio Xin, Bai Shao, Jiang Han Xia, Gan Jiang, Wu Wei Zi, Licorice, etc.) to warm yang and dissolve drink. The lung is the main source of qi, the kidney receives qi, the lung and kidney are of the same origin, and gold and water are related to each other. The formula uses Ginseng Mealybug San, Seven Flavors Du Qi Wan and Yang He Tang with addition and subtraction (Ginseng, Mealybug, Shu Di, Cornu Cervi Pantotrichum, Zi He Che, Cuscuta, Bacopa monniera, Xian Ling Spleen and Boneset). Decoct the above herbs together, remove the dregs and concentrate, supplement with gum, deer horn gum, pecan meat, honey and rock sugar to collect the paste. Take one spoonful in the morning and one spoonful in the evening with boiling water. In the following year, he complained that his asthma did not occur for one year, and his nasal congestion, runny nose and sneezing were significantly reduced. Case 2: Zhang, male, 60 years old, asthma for more than 20 years, in recent years, regardless of winter and summer, monthly attacks. In addition to asthma, he wakes up in the morning with sticky white sputum and not yellow, does not eat much, has lumbago, urinates frequently at night, even up to 6-7 times, or has nasal congestion and sneezing, has loose and soft stools, has a light red and vague purple tongue, has thin and greasy moss, and has a sunken and soft pulse. Comment: The evidence belongs to TCM that the lung, spleen and kidney are deficient, the fluid gathers into phlegm, water floods into drink and forces the lung Qi upward. The prescription is based on Su Zi Qi Lowering Tang, Ling Gui Zhu Gan Tang, San Zi Yang Yin Tang with addition and reduction (Zi Su Zi, Almond, Fa Han Xia, Orange Red, Poria, Qian Hu, Chuan Gui Zhi, Radix Angelicae Sinensis, Atractylodes Macrocephala, White Mustard Seed, Lycopodium, Scapularia, Bai Qian, Dry Ginger, Wu Wei), etc. to resolve the phlegm and treat the symptoms first; the formula is based on Jin Shui Liu Jun Decoction, Kidney Qi Wan, X Quan Da Teng, Yang He Tang, etc. (Prince’s Ginseng, Shu Di Huang, Radix Codonopsis, Astragalus, White Peony, Cinnamon, Sliced Pill, Cornu Cervi Pantotrichum meat, Chinese yam, coix seeds, bacopa monniera, cuscuta, codonopsis, spleen of the immortal soul, purple river carp, mealybug, antler slices, wolfberries, purple quartz, dong Hua, chicken’s inner gold, sand kernel, puzzle kernel, Jin Yan Yin ④褪怠怠ㄜ singular wounds (19) system of the Chinese government (γ) and the Chinese government (γ), the Chinese government (γ) and the Chinese government (γ).
