A four-phase physical therapy program is available for athletes with Type I, Type II and Type III injuries. Phase I: The focus is on pain relief and protection of the acromioclavicular joint through suspension immobilization (usually 3-10 days) and prevention of muscle atrophy. The author recommends starting with a closed-chain exercise of the scapula, which is easier to tolerate in the early post-injury period and allows the patient to focus on the scapular motion without causing increased discomfort. This exercise is a single scapular strength exercise, bearing only the weight of the upper extremity. Phase 2: Focuses on the recovery of full motor function, gradually promoting the recovery of muscle strength through isotonic exercises. Stage 3: Further strength exercises to improve dynamic stability of the acromioclavicular joint. Phase 4: Specialized sports exercises to prepare for the return to previous sports levels. All four phases of rehabilitation exercises need to be completed within 6-12 weeks.
