What types of leukemia are suitable for TCM treatment? Which stage is suitable for TCM?

  There is no doubt that Chinese medicine is effective in treating leukemia! However, the clinical results vary: some are effective, some are not; some are better, some are worse; some are faster, some are slower; some are for the leukemia itself, some are for the accompanying symptoms, etc. What is the reason for this? I would like to take this platform to talk to you and share with you my experience and insights on individualized stratified treatment, in the hope that TCM will bring more, better and faster results to leukemia patients.  There are two main types of leukemia: myeloid leukemia and lymphoid leukemia, both acute and chronic; myeloid leukemia is commonly granulocytic (acute, including promyelocytic), granulocytic and monocytic, erythroleukemic and megakaryocytic, etc.; lymphoid leukemia is commonly acute and chronic lymphoid leukemia and lymphoma leukemia, etc.; leukemia has an acute type transformed from chronic to pre-leukemic (now called “myelodysplastic syndrome”, MDS) into acute myeloid leukemia, etc.; in terms of age, there are adults and children, and among adults, there are geriatric leukemia types; in terms of disease course, there are primary and refractory relapses, and the risk level is low, medium, and high risk; in terms of treatment process, there are induction of remission and consolidation and intensification, and Bone marrow transplantation includes: pretreatment chemotherapy and/or radiotherapy, stem cell infusion and homing, hematopoietic and immune reconstitution, and the accompanying complications such as graft-versus-host disease; of course, there is much more than that in clinical practice.  I. Which type or subtype of acute and chronic leukemia is suitable for TCM-based treatment?  (1) As far as acute leukemia is concerned, promyelocytic leukemia can be cured with complete remission and long-term survival by simple application of Chinese herbal arsenic (intravenous versus oral preparation). In recent years, the combination of oral arsenic and the treatment with the addition and subtraction of the Chinese herbal medicine has been used to promote remission and long-term survival for patients who have relapsed after induction of differentiation and chemotherapy, and we have met with relapsed patients in Guangxi in the Chinese medicine clinic who came to the clinic in pairs and helped to achieve remission.  (2) Along with the accumulation of experience, gradually transitioned this drug to the treatment of myelodysplastic syndrome of high-risk to leukemia, combined with TCM discernment to administer the treatment of the formula to benefit the kidney and activate the blood and detoxify the toxin, clinical remission effect was obtained, it is this pure TCM arsenic agent combined with TCM discernment treatment made Guangzhou Huadu Ms. Z long-term sustained remission so far, more than 5 years, completely normal life.  (3) Elderly leukemia patients, especially non-leukemic (peripheral blood leukocytes do not increase) who are not suitable for chemotherapy, on the basis of strengthening supportive therapy, preventing infection and bleeding complications and protecting organ functions, Chinese medicine is given to support the main deficiency, detoxify and dispel evil, supplemented with conciliatory herbal treatment, clinical remission to a certain extent and control disease progression, and good quality of life with disease survival, some patients Of course, for the category of elevated leukocytes, if necessary, appropriate palliative oral chemotherapy (clinically tolerated) to control the tumor load while receiving TCM treatment, the same effect of survival with disease can be achieved; I remember a few years ago, the old Mr. H, who came to the clinic in Jiangxi province, had acute myeloid leukemia transformed by myelodysplastic syndrome. The old man and his family are confident that they will cooperate fully with the doctor’s treatment and strive for long-term survival and hope for a miracle “long life” with the disease, which is not impossible!  (4) Chronic lymphocytic leukemia is a category of chronic lymphocytic leukemia, mostly in the elderly, with slow development, and despite the continuous progress of modern medicine, it is still an incurable disease, and the conventional treatment in Western medicine is to obtain survival with the disease, unless a very successful allogeneic hematopoietic stem cell transplantation is implemented; in fact, as far as Western medical guidelines are concerned, non-progressive and non-high-risk individuals are not advocated for a single chemotherapeutic intervention; I am in the clinic for In my clinical practice, for this type of leukemia, I do not focus on lowering white blood cells and lymphocytes, but on stabilizing hemoglobin and platelet levels, promoting relative blood balance, delaying disease progression, preventing and treating infections and other complications, and keeping the patient in a state of normal life and working ability. Although the white blood cells are high around 20-30, sometimes fluctuating to 40-50 level, but the hemoglobin and platelets have always remained normal; a few days ago to follow up, the aunt told me: intermittently find western medicine specialist in charge of the previous doctor to give a review and evaluation, because the effect is good and its appreciation of Chinese medicine; especially the aunt told me that more than 10 years ago, many of the same period ” Auntie said that more than 10 years ago, many patients who were in the same period of time and simply pursued chemotherapy had already left, which left me a lot of thoughts!  Second, which stage or state of acute and chronic leukemia is suitable for TCM-based regimen?  (1) Acute leukemia is one of the most common tumors in childhood, and induction chemotherapy has a high rate of recent remission, but there is still a certain degree of relapse (especially in high-risk individuals), and after repeated strong combination or even high-dose chemotherapy, children in remission are prone to present such things as a less flushed face, frequent colds, easy sweating For more than 10 years, I have often seen children in need of recuperation after chemotherapy for leukemia in my TCM clinic, strengthening their constitution by benefiting Qi and nourishing Yin, promoting detoxification by promoting the detoxification of Dampness (Dampness and Poison) and Blood (Stasis and Poison), and promoting detoxification by strengthening the Spleen and regulating the stomach and intestines (Poison of Drugs and Poison of Disease). Parents often report that after taking herbal medicines, their children are in better health, their faces and complexions look better, and their diets and stools have improved, which makes parents happier! In view of the characteristics of children’s patients, intermittent Chinese herbal medicine should be given, alternating between internal and external use, so that children can easily accept it and get results.  (2) Adult (non-elderly) acute myeloid and lymphoid leukemia, although adults tolerate chemotherapy better and most patients can easily cooperate, but in today’s very emphasis on the benefits of increasing the dose and intensity of chemotherapy for the efficacy of the treatment, making some patients present physical incompetence and psychological fear, I often encounter such patients in Chinese medicine clinics: adult acute leukemia patients with inadequate chemotherapy or insufficient strength, not completed the prescribed course of treatment, and so on. In addition to improving the physical condition and relieving the symptoms of discomfort, I also provide timely reassurance and encouragement to build up their confidence in overcoming leukemia, which enhances their tolerance of chemotherapy from both physical and psychological aspects and makes their later treatment The treatment was individualized: timely sequential chemotherapy (intensity adjustment), delayed chemotherapy, suspended for a period of time to re-evaluate, actively involved in non-chemotherapy measures, or entered the stage of simple TCM comprehensive recuperation, etc.  A Guangzhou D lady, diagnosed with acute monocytic leukemia 10 years ago, was in remission after 2 courses of chemotherapy, and refused chemotherapy after the second half of remission due to the pain caused by the intensive stage of chemotherapy, and I treated her with TCM alone in the TCM clinic, together with dietary supplementation, and obtained long-term sustained remission and cure. In fact, as early as the late 1990s, I summarized a case of adult acute myeloid leukemia in which chemotherapy was inadequate but TCM was active in regulating the disease, and long-term survival was achieved.  Another woman, Ms. C, was diagnosed with high-risk adult acute lymphoblastic leukemia six years ago in the hematology department of a Western tertiary care hospital, and after the implementation of combined chemotherapy, she unfortunately contracted tuberculosis and had to terminate the sequential chemotherapy early; she sought my TCM help, and on the basis of anti-tuberculosis, I gave her TCM treatment in the TCM clinic to support the righteousness and dispel the evil: to benefit the qi and nourish the yin to support the righteousness, and to clear the residual toxins and fight the white. Although most of the adult acute lymphoblastic leukemia patients are high-risk patients, retrospective summary of my cases in the TCM clinic to help Chinese medicine interventions to identify and treat, there is no shortage of successful cases, in recent years, there are 5 to 6 patients have obtained good long-term results.  2, leukemia around the bone marrow transplantation period of Chinese medicine treatment 1) chronic granulocytic leukemia bone marrow transplantation after the biological relapse of treatment: Although bone marrow transplantation is one of the most promising effective means of curing leukemia, but there is still the problem of relapse after transplantation, once relapse, quite tricky and difficult! I once met a woman diagnosed with chronic granulocytic leukemia in Hunan Province in a Chinese medicine clinic, who was torn by the high price of a long-term targeted drug (Imatinib). Gene expression positive, the transplant specialist again recommended Gleevec targeted therapy to avoid relapse, the patient was very torn! The patient then sought the help of TCM doctors and was treated with active Chinese medicine interventions in my TCM clinic: to support the deficiency, to benefit qi and nourish yin, to cleanse residual toxicity and to administer conciliatory prescriptions, and after 3 months of TCM treatment, the fusion gene was found to be negative in two consecutive reexaminations, and the disease recurrence was prevented by the elimination of microscopic residual leukemia again. Believe it or not, she had a friend with a similar disease and treatment experience, who was also positive for specific fusion gene markers after transplantation, suggesting biological relapse, and brought up to seek my TCM treatment, and also obtained a negative result!  (2) Transplant-related chronic graft-versus-host disease (GVHD) is a headache for doctors and pain for patients after allogeneic hematopoietic stem cell transplantation, and once it occurs, Western medicine has a number of immunosuppressive drugs for such transplant medical problems, but the results are not satisfactory for some patients! The clinical symptoms of skin mucosa and muscle knuckles often affect the patient’s quality of life greatly, and even make the patient become anorexic; I once saw a Mr. Y who developed GVHD after pre-transplantation of leukemia: dry eyes, dry mouth sores, painful limbs, etc. He was identified and treated with the formula of nourishing yin and benefiting qi, cooling and moistening dryness, as well as harmonizing, and in just 2 weeks, the patient was able to The patient even said, “I see a ray of hope!  (3) Adverse reactions due to chemotherapy for leukemia As mentioned above, the main focus is on TCM adjuvant therapy for leukemia itself and the combination of its disease evidence-based treatment, with a view to obtaining control or remission of leukemia itself; more patients often need TCM help for delayed blood changes such as thrombocytopenia and/or anemia after chemotherapy for leukemia, because failure to obtain timely recovery can affect the next step Many clinical manifestations caused by chemotherapy and/or transplantation, such as poor health, susceptibility to infection, poor appetite, weakness, sweating, etc., affect the quality of life of patients, and according to my experience in TCM clinics, the appropriate time to intervene in the treatment and conditioning of Chinese medicine, together with diet and behavior management, can achieve good clinical results.
