First of all, it must be understood that saphenous varicose vein is only a manifestation, not a separate disease, varicose vein can be caused by saphenofemoral valve insufficiency, also may be caused by lower extremity deep vein valve insufficiency, some varicose veins secondary to lower extremity deep vein obstruction, saphenous vein surgery is not only ineffective but also harmful. In addition, varicose veins do not require surgery as soon as they are found, but the indications for surgery are serious aesthetic problems (of course, this is a matter of opinion), combined skin disorders (eczema, ulcers, bleeding, etc.), repeated superficial vein thrombosis, etc. The swelling of the skin is often not caused by varicose veins, but by deep venous lesions in such patients. Ulcers of the inner ankle are often not the only lesions of superficial varicose veins, but also need to be treated with combined lesions of deep veins or traffic branches. Therefore, varicose veins are often not a minor procedure that can be handled in a typical rural hospital. The treatment of varicose veins is divided into the treatment of the main trunk of the saphenous vein, the treatment of the varicose branches, and the treatment of other combined lesions. Laser treatment has become the main treatment for saphenous veins in recent years due to its simplicity, better cosmetic effect and exact results, and the cost is only a few hundred dollars more than traditional surgery, which is an obvious advantage. The treatment of varicose branches includes excision, sclerotherapy, loop sutures, punctal stripping, Trivex, etc. Our hospital mainly uses point stripping, which requires only a few incisions on the skin of no more than 5 mm in length to treat a large number of varicose branches, with minimal bleeding and excellent cosmetic results. Other aspects include shaping of deep vein valves, treatment of diseased traffic branches, and debridement and dressing of ulcers are also essential in the management of venous lesions of the lower extremities. In our hospital, we have been using lumpectomy for many years to dissect deep subfascial traffic branches to treat the diseased traffic branches, and combined with the corresponding ulcer treatment, it is very effective for the treatment of lower extremity venous ulcers.
