Abstract: Mucinous cystic tumors of the pancreas are a large group of cystic tumors of the pancreas, most of which are benign and usually have no obvious symptoms. The patient came to the hospital because of discomfort. After examination, it was found that mucinous cystic tumor of the pancreas was suspected, and after lumpectomy pancreaticoduodenectomy, as well as postoperative anti-infection, the patient’s lesion was excised cleanly and his condition improved, and a good treatment result was obtained.
Basic information】Male, 37 years old
Disease Type】Cystic tumor (mucinous cystic tumor of the pancreas)
Hospital】The First Affiliated Hospital of Guangzhou Medical University
Date of consultation】April 2021
Treatment plan】Surgical treatment (lumpectomy pancreaticoduodenectomy) + medication (cefoperazone sodium sulbactam sodium for injection)
Treatment period】8 days
Treatment effect] The lesion was removed cleanly after surgery, and the patient’s condition improved
I. Initial consultation
The patient visited the clinic because he was not feeling well, and the examination revealed a cystic mass of more than 5 cm in the pancreatic head area, resembling a common bile duct cyst, with a ductal structure next to the cyst, and the cyst and common bile duct were not connected. The patient was then given an EUS puncture and a cystic fluid pulling test, which could reach more than 1 cm, and the cystic fluid CEA was significantly elevated, and the patient was then advised to undergo surgery, and after discussion between the patient and his family, the patient was then given a lumpectomy pancreaticoduodenectomy.
II. Treatment process
Preoperatively, the patient was given ultrasound, CT, MRI and CT angiography so that the size and location of the tumor and its relationship with the surrounding blood vessels and whether there were important vascular variants such as the hepatic artery could be well known, and the assessment of heart, lung and kidney function was also improved so as to know whether the patient was suitable for lumpectomy pancreaticoduodenectomy. The postoperative pathology report confirmed the diagnosis of mucinous cystic tumor of the pancreas, and postoperative anti-infection treatment with cefoperazone sodium sulbactam was chosen.
III. Treatment effect
The patient was discharged from the hospital after 8 days of hospitalization with good postoperative recovery and good indexes. 2 weeks later, the patient returned to the hospital and no recurrence of the mass was found.
IV. Notes
We are glad that after the treatment, the patient’s lesion was removed cleanly, and his condition has improved, and his mental state is good. After the operation, the patient should choose light and easily digestible food when eating on weekdays, and can take in appropriate high-quality protein and vitamin-rich food, and can gradually resume the diet according to his physical condition. If discomfort occurs, such as indigestion, he can promptly consult a doctor and apply appropriate aids for digestion.
V. Personal insight
Mucinous cystic tumor of the pancreas is mostly round or ovoid, mostly located in the body or tail of the pancreas, usually with clear junction and intact cystic wall. The subsequent anti-infection and treatment achieved satisfactory results.
