As the main support structure in the middle of the face, the cheekbone has an important influence on the facial contour and appearance of a person. Western races have long, flat craniofacial faces and high cheekbones that add layers to the face and are considered a symbol of youth and beauty. Eastern races, on the other hand, have relatively wide cheeks and low, flat, narrow forehead orbits, which, when combined with high, protruding cheekbones, can appear disproportionate and lacking in beauty. High cheekbones affect the overall beauty: The cheekbones are located in the middle of the entire head and face, and are part of the overall beauty, which plays a role in setting off and corresponding to the glory of the entire face. The cheekbone is composed of bone tissue, muscle, subcutaneous and skin, and the supporting part of its shape is mainly bone tissue. The height of the cheekbone can directly affect the shape of the facial contour. The cheekbone and zygomatic arch can form a rhombic face shape, mainly manifesting as a sunken upper 1/3 of the face, a prominent middle 1/3, and a thin or unfilled lower 1/3, making the facial contour line not full, while the zygomatic body and zygomatic bone dysplasia can form a triangular face shape, affecting the appearance. This facial contour shape is not only related to appearance, but some people also only believe that facial features are related to fate. For example, if the cheekbones are too low, it means that the official luck is not good; if the cheekbones are too high, it means that the desire to control is strong, and some other very interesting statements. For example, we recently treated a female patient who had her cheekbones lowered because her cheekbones were too high and she couldn’t find a husband in the area. In fact, in the West, there is no such thing as high or low cheekbones, so perhaps this is the helplessness of the Easterners. As people’s demand for quality of life increases, more and more people are asking for cheekbone surgery. The first type is a simple chisel to reduce the outer contour of the cheekbone; the other type is a three-dimensional osteotomy to reduce the cheekbone through the shortening and movement of the bone. Zygomatic chiseling is generally suitable for patients with prominent anterolateral facial features. It is often performed by using an intraoral incision to expose the zygomatic bone under the periosteum and applying special instruments to chisel away and thin the part of the zygomatic bone that is designed to be removed before surgery, thereby reducing the external contour of the face and improving its shape. The biggest advantage of this surgery is that it leaves no scars after the surgery, and is seamless and unmarked. In contrast, zygomatic 3D osteotomy is considerably more difficult to perform and is suitable for patients with prominent zygomatic arches. In addition to the intraoral incision, an auxiliary incision is usually made in the hidden part of the hairline or a single coronal incision is made in the scalp, and the entire zygomatic bone is exposed by subperiosteal separation. This allows the patient to maximize the entire facial contour and shape, and even to achieve a “bone changing” effect. In addition to zygomatic bone reduction, for a small number of patients who have congenital or acquired diseases that result in a localized depression in the middle of the face due to zygomatic bone dysplasia or defects, or bilateral zygomatic asymmetry, zygomatic bone augmentation can be treated with autologous tissue transplants or prosthetic materials to fill the zygomatic area for the purpose of improving the appearance. It is worth noting that, like other plastic surgery procedures, zygomatic osteoplasty has the potential for complications, as there are sinuses in the cheekbones and facial nerves in the superficial areas. Of course, the aesthetic point of view of the physician is also an important factor in the patient’s decision to choose. It is recommended that patients seeking beauty must go to a regular hospital with a plastic surgery specialty, do not take their appearance or even life as a child’s play.
