The U.S. Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition-DSM-IV, anxiety disorders include the following five disorders: 1. generalized anxiety disorder 2.Acute anxiety disorder (also known as panic attack or cardiac neurosis). 3, social anxiety disorder (also known as social phobia or phobia or square phobia or barefaced phobia). 4, post-traumatic stress disorder. 5, obsessive-compulsive disorder. Generalized anxiety disorder is one of the more common anxiety disorders. The lifetime prevalence of generalized anxiety disorder in adults is estimated at 4.1%-6.6%, with female patients being twice as likely as males. Generalized anxiety disorder (GAD) is a type of anxiety disorder that is characterized by anxiety and nervousness that lacks a clear object and specific content, with significant autonomic symptoms, muscle tension and motor anxiety. Patients suffer from unbearable and unrelieved distress. The duration of the illness is at least 6 months. Excessive worry and anxiety caused by other disorders should be ruled out before diagnosing generalized anxiety disorder. There are three main manifestations of generalized anxiety disorder, which can occur simultaneously or can be dominated by a particular symptom cluster. 1. Psychogenic anxiety or floating anxiety is manifested as: patients feel nervous and uneasy for a long time. When doing things, they are distracted and impatient; when interacting with others, they are nervous and eager and extremely unsteady; when encountering unexpected events, they are panicked and disoriented, and their memory goes to bad places. Inexplicable or factually inconsistent anxiety, fear, nervousness, fear of flying disasters. 2.Motor restlessness: mainly includes fidgeting, rubbing hands and feet, shaking limbs, generalized muscle tension pain and tremor of tongue, lips and finger muscles, etc. 3.Somatic symptoms: Patients with generalized anxiety disorder have obvious somatic symptoms, with pain and fatigue being more prominent. Somatic symptoms can be accumulated in many systems of the body, such as respiratory, cardiovascular, digestive, neurological, and urinary systems. The main symptoms are: panic, chest tightness, shortness of breath, dizziness, excessive sweating, facial flushing or pallor, taste, foreign body sensation in the throat, gastrointestinal discomfort, nausea, abdominal pain, bloating, diarrhea, frequent urination, pain in various places, muscle tension, etc. Some patients can develop sexual dysfunction such as premature ejaculation, menstrual disorders and lack of libido. Treatment: 1. Treatment in the acute stage: control the symptoms and then achieve clinical cure. This period is mainly based on drug treatment, the treatment time is 10-12 weeks. The duration of drug treatment can vary greatly depending on the mechanism of action, with the onset of action generally starting in 1-2 weeks and extending to 2-4 weeks in severe cases. FDA-approved medications for the treatment of generalized anxiety disorder include venlafaxine, duloxetine, paroxetine, escitalopram, and buspirone. The drugs approved by SFDA in China for the treatment of generalized anxiety disorder are venlafaxine, buspirone and trazodone. Of course, in clinical practice, other 5-hydroxytryptamine reuptake inhibitors such as fluoxetine, sertraline, citalopram, fluvoxamine and benzodiazepines such as alprazolam and diazepam are often used in the treatment, and as long as they are used appropriately, the efficacy achieved is also very satisfactory. 2, consolidation period treatment: at least 2-6 months, in this period the patient’s condition is unstable and the risk of relapse is higher. 3. Maintenance treatment: Generalized anxiety disorder is a chronic and highly relapsing disease that requires maintenance treatment for at least 12 months to prevent relapse.
