Cataract surgery after.
A 77 years old senior patient (female) had a cataract removed (left eye) and an IOL fitted on March 1, 2012. The surgery was very successful and everything is normal one month after the surgery till now. A brief description of the surgery is provided for reference.

II. After the surgery.
1. I was hospitalized for surgery, and one day before the operation, the doctor did a detailed examination and asked for medical history. It turned out that Dr. Hao thought that both multifocal adjustable crystals and monofocal crystals were suitable for me and recommended the former. However, after repeated consideration, I chose the monofocal crystal, not because of the price, but because I did not know much about multifocal adjustable crystals and was afraid of the danger of using the new technology. Now I have some regrets. As a patient with good health, good fundus condition and financial ability like me, since I had eye surgery, I should choose multifocal adjustable crystal and enjoy the welfare brought by high technology.
2. I usually have normal blood pressure and no cardiovascular disease, and my blood pressure was 120/90 when I was admitted to the hospital, but on the morning of the surgery, my blood pressure was 110/145 (the doctor said that the surgery could still be performed). After entering the operating room, when the doctor led me to the operating table, I kept saying, “I’m not nervous, cooperate with the doctor’s surgery”. I got off the operating table in about five minutes and was in and out of the operating room in less than fifteen minutes, and the surgery went very smoothly. The surgery was only painful for a few seconds (just like a normal injection), and it was so fast!
I said I was not nervous, but I heard from the doctor later that my blood pressure rose to 175 on the operating table (the anesthesiologist took care of it with a little treatment). So when I got off the operating table, I had a headache in the upper part of the operating eye for about 10 seconds, which was short and intense (probably due to the elevated IOP) and was different from a cold headache, which is a long, slow pain at the top of the head. After the operation, I entered the ward to measure my blood pressure, which had dropped to 160/90. Two hours later, my blood pressure was normal, 140/90 and 120/90 respectively.
III. Postoperative sensation.
1. On the night after surgery, I felt a golden light in the operated eye (blue base light plus star yellow light), which did not open the operated eye at that time, but disappeared quickly, without pain or other discomfort.
2, the day after surgery, the doctor removed the eye patch after examination, the visual acuity of the operated eye rose from 0, 08 to 0, 8, the unoperated eye is still 0, 6, intraocular pressure 17, 9, 15, 1, the operation is very successful, the doctor and the patient are very happy. Only the operated eye was slightly red and there were some flowers in front of the eyes, the doctor answered the reasons are.
(1), vitreous clouding in the operated eye (it has been more than two years).
(2), just operated with anesthetic, eye medicine, etc..
But maybe my original eye disease is cataract, dry eye, flash, presbyopia, this surgery only solved the cataract, improve vision, and can not solve the flash. Later, I put on my old presbyopic glasses again and my eyes were not blurry, but the cataract was cleared and I could see very brightly.
3. On the second day after surgery, at 2:00 in the middle of the night, I got up because I couldn’t sleep and looked at the clock on the wall opposite the bed. For a few minutes, I couldn’t see at all in my left eye (the operated eye) and there was only some light in my right eye.
4. In the first two days after surgery, the operated eye was still a little red, but it healed quickly. But for the first two days, I felt that there was a foreign body in my eye, and I couldn’t open my eyelid, so I kept turning it over with my hand (as if it was sticking). I opened my eyelids and saw that there was a wound of about 3mm above the operated eye (at 12 o’clock), which was slightly red. I can read, watch TV and write at home. I feel so happy at this moment! It turns out that the colors of nature are so beautiful. After watching color TV for thirty years, I now feel that its colors are so colorful. The blue sky is so clear, and my home is suddenly much brighter. It was great to have the cataract removed!
5. I went to work one week after the operation and had the following feelings while walking.
(1) When the sky was clear, I felt that the light was too strong for my eyes, so I put on sunglasses.
(2) I felt some lightness under my feet when I walked, but now I am slowly getting better.
(3) Sometimes I feel that both eyes are a little wet, but there are no tears.
6. One month after the operation, the vision of the naked eye has become better (my vision also feels the same), from 0.8 to 1.0 in the operated eye and from 0.4 to 0.6 in the unoperated eye, I wonder if this vision can be stable.
Fourth, a few feelings.
1. Although cataract removal is a very mature surgery, for everyone, “eyes are life”, we have to consider it very carefully before deciding, and many people are very afraid. After this surgery, I feel that as long as I am in good health and my eye is free of disease, the success rate of cataract surgery is very high and the risk is very low.
2. Since they are determined to have cataract surgery, most patients do not consider price as their first choice, and they do not choose adjustable multifocal lens because they do not understand it. It is not because they are afraid of spending more money, but mainly because they heard people say that “using complex multifocal crystals is not easy to adjust after surgery and there are serious consequences if they change the crystals again”. I would like to suggest if we should strengthen the promotion and explanation of multifocal adjustable crystals, so that if the patient is financially capable and suitable to use multifocal crystals, the doctor can adopt them directly. Since they have to choose between multifocal and monofocal crystals, patients will choose monofocal crystals from a conservative point of view, so it will be difficult to promote and improve multifocal crystals.
1.When do I need cataract surgery?
For a long time, the people have been circulating this saying that cataract can only be operated when it is fully mature or even invisible, which is a wrong view. With the popularity of microsurgery, vision loss is no longer the only indication for cataract surgery. Generally speaking, as long as cataract affects daily work and life, surgery can be considered. Some patients may have good vision on examination, but many scenes in their daily life are not as black and white as their vision chart. As long as they have symptoms such as photophobia, glare, and decreased contrast sensitivity, they can consider cataract surgery to improve their quality of life. If the cataract is delayed until it is mature or even overly mature, not only will it cause secondary glaucoma and uveitis, but the surgery will be much more difficult and the surgical complications will increase accordingly. The author of this article, as a senior intellectual, is still late for surgery (77 years old, bare eye vision 0.08, corrected vision 0.4), which shows how widespread the false statement “cataracts should be done only when they are mature” is.
2. Does cataract surgery hurt?
Generally speaking, adults who can cooperate with cataract surgery only need local anesthesia or even surface anesthesia, and generally do not feel any pain during the surgery. After the surgery, the doctor will inject anti-infection drugs under the conjunctiva of the operated eye, and some people will feel pain for a short time.
3.What kind of IOL do I need to implant?
IOLs are implanted in the eye during cataract surgery to replace the artificial lens removed due to clouding. According to current clinical observation, IOLs have stable performance and can last for more than 70 years after implantation, and do not need to be replaced without special circumstances. According to the function, there are four types of IOLs in clinical application: rigid IOL, foldable IOL, adjustable IOL and special IOL. Hard IOLs have been widely used, foldable IOLs are currently the mainstream, adjustable IOLs are undergoing more cutting-edge research and application, and special IOLs are highly individualized and targeted. Each IOL has its own characteristics and there are no absolute advantages or disadvantages, and they need to be jointly developed by the surgeon according to the actual situation of the patient.
