What should I do if I have a large prostate enlargement?

  Zhang Tai-Tai, 77 years old, began to have urinary problems and increased nighttime urination 10 years ago. She had to get up two to three times every night. In the last five years, the number of nighttime urination has increased to four to five times, and there is also the phenomenon of blood in the urine from time to time. At first, Zhang Cai did not pay attention to it, thinking that it was a normal phenomenon after getting older, but as the symptoms worsened, he gradually began to be alerted to the possibility of other problems. The first thing you need to do is to go to a famous hospital in the province and find out that the prostate is quite large, more than 20 times larger than normal, and more importantly, there is a test item PSA results are worrying: TPSA > 50ng/ml, highly suggestive of the possibility of prostate cancer. The actual results of the biopsy are “benign prostate hyperplasia”.  The results came out, and the family’s tense nerves were temporarily relaxed. The patient was discharged from the hospital with repeated hematuria, which brought a lot of stress to the patient. In order to completely get rid of the difficulty in urination and hematuria, the patient was admitted to our urology department.  The CT examination showed that the size of the prostate gland was 11*10*8cm, which was calculated to be 457 ml. This is the largest prostate gland ever seen in our hospital and is quite rare in Hunan Province. The CT also showed that the prostate was so large that it had taken over the position of the bladder, pushing it upwards and compressing it. It is the pressure of the prostate on the bladder and urethra that causes the patient to experience frequent urination and difficulty in urination; due to the large size of the prostate, the surface blood vessels are hyperplastic and varicose, which can easily rupture and bleed, and is also the cause of the patient’s repeated hematuria. After careful discussion in the department, the patient’s surgical indications were considered clear and the surgical conditions were met.  After perfecting the preoperative preparation, he was operated on. The surgical procedure was also tumultuous, as the surgical space behind the pubic bone was already quite narrow because of the deformed growth of the prostate. As the enlarged prostate has two lateral lobes, one of which is near the ventral side and relatively easy to deal with, Professor He Le Ye decided to start with it first and remove it first after careful peeling. This way, the surgical space was increased and the other lateral lobe was easier to deal with. Finally, the hyperplastic gland was removed and weighed, and the needle stopped at 450g. Although we were prepared for this before, we all sighed at the prospect of such a large prostate in front of us. After careful treatment, the patient recovered relatively well after surgery. The catheter was removed 3 weeks after the surgery and the patient urinated smoothly and was discharged from the hospital.  The normal adult male prostate weighs 20 g. From the age of 40, the prostate starts to grow mildly, and then increases significantly from the age of 60 to 70. The prostate gland is more than 40 to 100g, rarely more than 200g. we have not seen reports of more than 400g in our province. The most important thing is that you can get the most out of your life.
