How should elderly cardiovascular patients be medicated?

  This is a real case from an outpatient clinic.  An 86-year-old gentleman with an old cerebral infarction, who also suffered from hypertension and diabetes, was still relatively stable, with some carotid atherosclerotic plaques on examination, but everything else was fine, but the old gentleman had to take a large amount of medication every day, and many of the key ones were not very necessary. Out of medication principles, I streamlined the old man’s daily neurological medication to atorvastatin and aspirin, telling him to take one of each every day would be fine, and as a result …… I was complained about by his family for not giving medication to the old man! It is really a bit depressing.  We can look at the old man’s previous medication: 12 heart and brain capsules 3 times a day, 4 at a time; 6 earth kinase capsules 3 times a day, 2 at a time; 12 heart and brain tablets 3 times a day, 4 at a time; 9 blood lipid Kang capsules 3 times a day, 3 at a time; 9 blood Cetron capsules 3 times a day, 3 at a time; 2 atorvastatin (10mg)’s 2 times a day, 1 at a time. 1 tablet at a time.  We can simply calculate that we need to take 12+6+12+9+9+2=50 tablets a day! And that’s just for neurology! He is also taking 2 antihypertensive drugs, 1 coronary heart disease drug, 2 hypoglycemic drugs, 1 drug for prostate enlargement, a total of more than 60 pills per day!  The drugs are indeed all common drugs for cardiovascular diseases, but the key is that the old grandfather is 86 years old, usually living with his partner, the children are not around, the old man does not remember whether he has taken the drugs every day, the daily drugs are not right. He takes a lot of medicine every day, and according to his own words, he is full of medicine. Every month, he has to take a large book bag to the hospital to get his medicine, filled to the brim. Moreover, some of these pCms contain multiple ingredients, and some of them are duplicated when taken at the same time.  What does the modified medication look like?  1 tablet of aspirin (100mg), + 1 tablet of atorvastatin (20mg) per day = 2 pills.  So why can’t the patient’s family understand?  When I was confronted with family members who complained, they said, “That’s what the doctor prescribed before, and we’ve been taking it for years, so why is it 2 tablets with you? What if our elderly people don’t take the medicine and get sick?” I calmly told them: the more medicine you take, the better, but the better it works. I have no intention to judge the previous doctor’s treatment, but whether any treatment is effective should be based on, this basis is not a doctor said good is good, but in the treatment of millions of patients in a controlled study of the objective law, in other words, this is millions of patients with their own changes and even life for the objective laws of medicine, this has a special term, called “evidence-based medicine”. For the secondary prevention of ischemic cerebrovascular disease, the treatment with such evidence is divided into three main areas, referred to as the “ASA” three cornerstones. The first A refers to antihypertensive agents, the S refers to statins, and the second A is antiplatelet agents, which are also included in the Chinese Guidelines for Secondary Prevention of Ischemic Stroke/Transient Ischemic Attack.  Of course, these treatments are fundamental to prevent recurrence of cerebral infarction. Other aspects are also important, such as lifestyle changes, quitting smoking and limiting alcohol, being active, and maintaining a good mood. Like this old man who also has diabetes, he also needs to control his blood sugar well. Comprehensive treatment is the only way to get good results.
