The fallopian tube is one of the female internal reproductive organs, the tube that carries the eggs and is the site of fertilized egg union. Its incompetence or incompetence is the main factor leading to infertility in women, accounting for 1/3 of female infertility factors.
The analysis is as follows.
1. Object and method.
Subjects.
All 50 cases in this group were outpatients, married, aged 20 to 39 years, with 15 cases of incompetent fallopian tubes and 35 cases of incompetent tubes, with a minimum of six months and a maximum of 15 years of marriage, and with a history of never having a uterine operation in 4 cases and a history of uterine operation in all others.
Methods.
Combination of Chinese and Western medicine.
Western medicine.
Physiological saline 20ml~40ml, α~chymotrypsin 5mg, dexamethasone 5mg; Atropine 0.5mg can be added to antispasmodic in cold weather to rule out the illusion of tubal incompetence due to tubal spasm; 0.5% lidocaine 2~3ml can be added to relieve pain and reduce resistance when the patient is particularly nervous.
Methods.
It is even simpler than the tubal lavage method, that is, after the diagnosis is clear, a 20ml syringe connected to the end of the tubal lavage device can be used to inject the drug. Pressure is added when the resistance is high, and the patient can be allowed to elevate the hips when there is more fluid outflow. It starts 3~7 days after clean menstruation, once every 2~3 days, stops until the middle of menstruation and is repeated in the next menstrual cycle.
Chinese medicine.
To clear heat and dampness, activate blood circulation and resolve stagnation. Use Red Vine Soup to retain the enema: 30g of Red Vine, 30g of Fructus septicae, 30g of Gong Ying, 30g of Di Ding, 30g of Danshen, 20g of Honeysuckle, 10g of Fritillaria, add 100ml of decoction of the above herbs with water and thick juice, the temperature of the medicine is about 40℃, enema after stool, rest in bed for 20 minutes after enema, once a day or once every other day, a course of treatment for 30 days, without interruption during menstruation.
Discontinuation time.
The discontinuation time is judged according to the treatment results, with a maximum of 10 applications of western medicine and a minimum of 2 applications; the longest important application is 90 days and the shortest is 20 days.
2.Efficacy determination.
Mainly by the doctor’s hand and patient symptoms as a reference, such as the doctor feels little resistance, the patient’s symptoms are less than before, take a hydrogen peroxide lavage, and under the ultrasound to observe the direction of bubbles and uterine rectal sink fluid collection.
The resistance is small, the symptoms are light, the bubbles go to the umbrella section smoothly, and the uterine rectal sink fluid is mostly cured. If the resistance is large, the symptoms are not reduced, the bubbles do not go to the umbrella section, and the fluid in the rectal sink of the uterus does not collect, it is ineffective. Between the two is ventilation but not patency. It can be compared with the initial state of the patient.
3. Observation of therapeutic effect.
There were 5 cases in which the fallopian tubes were incompetent and 31 cases in which the tubes were incompetent and became patulous. The remaining 4 patients felt less symptomatic than before. It was found that there was a negative correlation between tubal incompetence and patency and the age of the patients, especially the time since the last uterine operation.
4. Discussion.
Due to the high development of modern assisted reproductive technology, infertility patients can be treated more quickly. However, the high price has discouraged many patients and has caused many physicians to overlook the simple and economical experimental treatment. The advantages of this method are economic simplicity, operability, little pain, and 72% cure rate, but the disadvantage is that the course of treatment is long and some patients cannot adhere to it every day. As long as the medication is adhered to, most patients have the possibility of being cured. Through the above observation and treatment, this method is considered to be reliable and worth promoting.
