About the characteristics of PPH surgical treatment

PPH is the abbreviation of ProcedureforProlapseandHemorrhoids, which directly translates as the (treatment) method for prolapse and hemorrhoids, and is known as anastomotic suprahemorrhoidal circumferential hemorrhoidectomy in China, which means that the mucosa and submucosa of the lower rectal wall are circumferentially excised above the prolapsed internal hemorrhoid near the upper edge of the internal hemorrhoid, and the distal and proximal ends are anastomosed at the same time as the excision. The mucosa is anastomosed so that the prolapsed internal hemorrhoids and mucosa are suspended and pulled upward and no longer prolapse, and at the same time, because the superior hemorrhoidal artery located in the submucosa is cut off, the blood supply to the hemorrhoids is reduced after surgery and the hemorrhoids tend to become smaller, thus achieving the purpose of hemorrhoid treatment. Therefore, the exact name of PPH hemorrhoid surgery is “circumferential resection and anastomosis of the lower mucosa and submucosa through the anorectum and repositioning and fixation of the anal cushion to treat hemorrhoids”. The indications for PPH surgery are: III and IV degree internal hemorrhoids with circumferential prolapse and II degree internal hemorrhoids with recurrent bleeding; anterior rectal bulge and endorectal prolapse leading to functional outlet obstruction type constipation. PPH preoperative preparation: routine blood test, coagulation function, electrocardiogram if necessary; oral medication to cleanse the intestine the night before surgery or enema in the morning of the surgery; diet abstinence in the morning of the surgery for those who use intralesional anesthesia or general anesthesia. Postoperative management of PPH: Observe for bleeding (including early and delayed bleeding). Rectal mucosal protector can be placed to facilitate wound healing and defecation. Give appropriate treatment for urinary retention and pain. Appropriate prophylactic antibacterial drugs are appropriate. Food can be eaten after recovery from anesthesia and irritating food should be avoided. Advantages of PPH procedure: Cure or significantly change the preoperative symptoms with outstanding effect. Less damage and less pain. This is a very important advantage because the anal region is very painful after traditional surgery, and some foreign scholars describe this pain as painful like draining glass slag, and claim that this advantage alone is enough for patients to accept this procedure. It is more in line with human anatomy and physiology. Since the procedure is performed over the hemorrhoids, the anal cushion is preserved intact so that the normal defecation and exhaustion functions of the person are not affected, while avoiding the complications of anal stricture. The operation time is short and the patient recovers quickly, saving hospital time and thus some money.
