The culprits that tormented the 60-year-old

On this day, the spinal neurosurgery clinic of the First Affiliated Hospital of Chongqing Medical University (CMU) admitted a 62-year-old woman surnamed Yu to the clinic with “thoracic 8-9 vertebral canal occupancy”. Three months ago, the weakness and numbness in both lower limbs had increased compared to three years ago, and the weakness and numbness gradually increased from the navel down to the back of the foot, making it difficult for her to stand up. Professor Yan Yi of the Department of Spinal Neurosurgery at the First Affiliated Hospital of Chongqing Medical University learned more about Yu’s condition. It turned out that she had experienced numbness in both lower limbs three years ago, and was diagnosed with “lumbar disc herniation” by the local hospital, which improved after physical therapy. Prof. Yan made a preliminary diagnosis from Yu’s examination report: thoracic 8-9 intravertebral nerve sheath tumor; primary hypertension grade 3. Yu’s condition required a thoracic 8-9 intraspinal tumor resection as soon as possible, and the patient and her family agreed to the surgery under Professor Yan’s advice. Prof. Yan introduced that the most common first symptom of intradural nerve sheath tumor is radicular pain, followed by sensory abnormalities and motor disorders. The pain of tumor in upper neck segment is mainly in cervical neck, and occasionally radiates to shoulder and upper arm; the pain of tumor in cervicothoracic segment is mostly located in the back of neck or upper back, and radiates to one or both sides of shoulder, upper limb and chest; the tumor in upper thoracic segment often manifests as back pain, which radiates to shoulder or chest; the pain of thoracic segment is mostly located in thoracic and lumbar region, and can radiate to abdomen, groin and lower limbs. The pain of tumors in the thoracolumbar segment is located in the lumbar region and may radiate to the groin, arm, thigh and calf. The pain of lumbosacral segment tumors is located in the lumbosacral region, buttocks, perineum and lower extremities.
