There are obvious regional differences in the incidence of prostate cancer, with a high incidence in Europe and the United States, where it is the first malignant tumor to develop in men and the second most common malignant tumor death in men. In the past, the incidence rate in China was low, but in recent years, with the improvement of living standard and medical care as well as the increase of people’s average life expectancy, the incidence rate of prostate cancer has increased exponentially. The lack of characteristic clinical manifestations of prostate cancer makes about 40% of prostate cancers are already advanced at the time of diagnosis and lose the chance of radical surgery. Therefore, endocrine therapy represented by orchiectomy plus androgen receptor blockers is a common treatment method used by urologists in China. This approach is effective for the vast majority of patients with advanced prostate cancer, where the patient’s PSA (prostate-specific antigen) decreases and the primary tumor foci and metastases shrink or disappear. However, one of the signs that these effective patients will one day progress again is that their PSA rises again. We call this condition destructive-resistant prostate cancer. Desmoplastic-resistant prostate cancer is a difficult part of prostate cancer treatment. Second-line endocrine therapy or chemotherapy can be used for this type of patients, and there are multiple treatment options to choose from based on the doctor’s experience, the patient’s physical condition and economic situation, and a good treatment plan can maximize the efficacy, prolong the patient’s survival and reduce the adverse effects in treatment. The Department of Urology of East China Hospital of Fudan University has devoted itself to the basic and clinical research of desmoresistant prostate cancer for many years, and has accumulated rich experience in the diagnosis and treatment of desmoresistant prostate cancer and formed corresponding scientific and technological achievements, which are at the leading level among the peers.
