Facial palsy is categorized into central and peripheral, with central usually secondary to cerebrovascular disease and peripheral usually referring to primary idiopathic facial nerve palsy. Idiopathic facial nerve palsy is a common mononeuropathy of the brain and is the most common cause of facial paralysis, with an incidence of (11.5-53.3)/100,000 cases. The exact etiology of the disease is unclear and is generally thought to be related to viral infection or inflammatory response. It usually starts acutely and tends to peak in about 3 days, presenting as unilateral peripheral facial paralysis. The disease is self-healing, but early treatment can speed up the recovery of facial paralysis and reduce complications. Facial paralysis mainly manifests as unilateral peripheral facial paralysis, and the main symptoms are: weakness of closing eyes, frowning, puffing out cheeks, baring teeth and shutting up on the affected side, as well as drooping of the corners of the mouth; it may be accompanied by pain behind the ear on the same side or pain in the mastoid process. The symptoms usually appear suddenly and should be treated promptly by a doctor to distinguish primary and secondary facial paralysis, make a clear diagnosis, and then carry out systematic treatment. Drug therapy: For peripheral facial nerve palsy caused by facial neuritis, antiviral therapy can be used if it is a viral infection, and at the same time, injections or oral therapy with nerve nutrients, B vitamins and other drugs can be used. Glucocorticoids can be used for short-term treatment for patients without contraindications. Traditional Chinese medicine treatment: early use of heat and detoxification, wind and collaterals of traditional Chinese medicine, after the acute phase of the use of blood circulation, collaterals and qi and other methods, together with Western medicine can effectively alleviate the symptoms and speed up the recovery. Acupuncture treatment: Early acupuncture treatment can reduce the inflammatory reaction and promote the recovery of nerve function, and it is recommended to be treated by professional acupuncturists. Physical therapy: commonly used ultrashort wave, low and medium frequency electrotherapy, laser, drug introduction, etc., which has a certain auxiliary therapeutic effect. Rehabilitation therapy: Hot compresses can be applied to the affected side to promote local blood circulation. When the facial muscles start to recover, it is necessary to do muscle training of facial muscles, mainly training the expression muscles, do opening eyes, wrinkling forehead, sucking, cocking lips, whistling, pouting lips, opening mouth and other actions, each time about 20min, 1-2 times a day, until the final recovery. At the same time of actively adopting medication, nursing care should be strengthened to prevent direct wind blowing on the face; when the patient has incomplete eyelid closure, attention should be paid to the protection of the patient’s eyes, and an eye mask should be used when necessary; attention should be paid to rest, preventing overwork; and the diet should be a list of things to be eaten, avoiding spicy and oily diets. Therefore, most of the patients with idiopathic facial nerve palsy can be cured by themselves, but due to the difference of patient’s age, physique, and underlying diseases, some of the patients will be left with sequelae. Therefore, after the symptoms are found, it is necessary to seek medical treatment in time, make a clear diagnosis, and adopt comprehensive treatment means, which can effectively reduce the sequelae and shorten the time of recovery.
