Due to the large area of the injured skin and the many exposed nerve endings, microcirculation disorders and aseptic inflammation are likely to occur, resulting in localized swelling and pain. The patient was treated with RICE+POLICE, and the discomfort was reduced and the wound recovered well. Initial Consultation The patient is a sports enthusiast who came to the clinic because he said that on the way back from a 100-mile hill-climbing race, he was cut off by a battery car due to poor vision, and suffered abrasions on his hands and legs, accompanied by swelling and pain. Physical examination showed: T: 36.2℃, P: 79bpm, R: 19bpm, Bp: 128/67mmHg, mental clarity, no cranial deformity or mass, bilateral pupils are equal in size and rounded, light reflex is normal, neck is soft, thorax has no deformity or tenderness, no obvious abnormality of the heart and lungs, abdominal flatness and softness, no tenderness, no tenderness of the spine and pelvis, tenderness in the upper part of the left thigh and the left calf, no obvious swelling, no deformity or bone rubbing. There was no deformity as well as bone rubbing sensation. He was admitted to the hospital with a preliminary diagnosis of multiple contusions and abrasions, which needed to be confirmed by a progressive examination. Second, the treatment process After admission, X-ray examination was performed, and treatment was carried out using the RICE treatment method for the acute phase of soft tissue injuries, i.e., braking, cold compresses, compression bandages, and elevation of the affected limb. First, the patient was instructed to lie on the trauma and keep the injured part braked to avoid bone and joint damage; the wound was flushed with saline solution and wiped with a sterile cotton swab to remove the wound dirt and prevent skin bacteria and tetanus; next, iodine povidone was applied to the affected area to sterilize and disinfect it, and a thin layer of petroleum jelly was lightly applied; the affected area was wrapped with a sterile bandage (compression bandaging); after the bandaging was completed, an ice bag was given to apply it to the affected area (cold compress); and the muscle strain was reduced. After the bandaging was completed, an ice pack was placed on the affected area (cold compress) to reduce muscle strain; and a pillow was placed under the affected limb (to elevate the affected limb) to promote blood return and reduce the swelling of the affected area. Subsequently, the patient was informed of the need for the POLICE treatment method for the rest of his life, i.e., protection, increased loading, cold packs, compression, and elevation of the affected limb. First, in the first week after the injury, the “RICE principle” was followed to rest the injured tissue and reduce swelling. Secondly, in the second to third weeks after injury, gradually resume movement (increased loading) of the limb and the affected joint. Lastly, return to sports gradually over several weeks and months until the injured tissues return to normal. Treatment effect After admission, the patient’s pain in the affected area was reduced and relieved by the treatment of RICE + POLICE. On the 3rd day of admission, the patient was discharged from the hospital as the pain in the affected area was alleviated. 1 week later, the localized abrasion had been scabbed without oozing, and 2 weeks later, the epidermal scabs were gradually falling off, and the scabs did not leave any obvious scars. The affected limb was free to move, and the overall recovery was good. The patient was very satisfied with the treatment and the therapeutic effect. IV. Precautions We are glad that after the treatment, the patient’s discomfort is reduced and his body gradually recovers and can return to the normal state. The patient should closely observe the condition. If symptoms such as localized pain and swelling reappear, it suggests that the disease may change or aggravate, and prompt medical attention is needed. Pay attention to the reasonable arrangement of sports, bedtime, can be appropriate to strengthen the limbs of the activities, such as appropriate elbow, knee flexion and extension activities; recovery, need to strengthen the limbs of the functional exercise, step by step, not to carry out strenuous exercise and heavy manual labor, exercise, pay attention to the affected area protection, to avoid re-injury.
