It is one of the most serious complications of diabetes. The diabetic foot develops rapidly and improper management can lead to a rapid deterioration of the patient’s condition, severe disability, and even death. Domestic statistics report that among all causes of skin injury, the proportion of diabetic foot increased from 4.8% in 2002 to 35% in 2010, ranking first. And according to foreign epidemiological surveys of diabetic foot, about 15% of diabetic patients will develop diabetic foot in their lifetime. Referring to this figure, and considering the huge base of diabetic population in China (nearly 100 million people), in the future, China will face a huge pressure of increasing number of diabetic foot patients. Foreign research data show that diabetic foot accounts for 47% of all hospitalizations for diabetes-related problems. The medical costs associated with diabetic foot ulcers and amputations are enormous. In the United States, these costs account for nearly half of all diabetes-related costs. Studies have also shown that the 5-year mortality rate of patients with amputations due to diabetic foot is even higher than the 5-year mortality rate of some malignancies such as colorectal cancer. Therefore, the clinical problems of diabetic foot in western developed countries are receiving more and more attention. A well-developed medical system including clinicians, podiatrists and specialist nurses has been formed. In addition to the lack of experienced physicians and nurse practitioners, there is also a large gap in the concept and awareness. At present, both doctors and patients pay insufficient attention to the diabetic foot, and the treatment of diabetic foot disease is not standardized and scientific, resulting in minor illnesses becoming major disasters, and some small wounds eventually develop into amputations or even death because of untimely medical treatment or improper disposal. At the same time, the awareness of diabetic foot and its hazards among all parties in society has yet to be improved, so some related systems and norms need to be improved; such as the training of related talents, the establishment of the scope and standard of podiatry service charges, the establishment of medical and social education system, etc. As a province with a large population and a region with a high prevalence of diabetes, our province faces the same problem. From the definition, we can see that diabetic foot is a multidisciplinary pathology, such as metabolic endocrinology, dermatology, orthopedics, vascular surgery, infection, orthopedic surgery, and radiology and other clinical specialties, so a good podiatrist needs to have a variety of professional knowledge and skills, and at the same time, the treatment of complex diabetic foot also requires the cooperation of multiple specialized departments, which places a great demand on the admission of diabetic foot patients This places high demands on the overall level of medical institutions that treat diabetic foot patients. As a leader in the field of endocrine diabetes in the province, the Department of Endocrinology of the Provincial Hospital has done some solid and serious work in the prevention and treatment of diabetic foot in recent years, and has done a lot of work in basic treatment and infection control, local disposal of traumatic surfaces, intracavitary molding of lower limb arteries to reconstruct blood supply to the extremities, and screening of high-risk factors of diabetic foot; meanwhile, as early as 2006, it established a department including hand and foot surgery, In March 2006, we completed the first case of intracavitary molding of long-segment lesions of the infrapopliteal artery of the diabetic foot in our province, which opened up a new path for the treatment of diabetic foot and achieved good social benefits. It was widely praised and reported by the media. Based on the above work, in order to better carry out the prevention and treatment of diabetic foot, this month, we will set up a specialized diabetic foot clinic in the East Hospital of the Provincial Hospital, which is the first diabetic foot specialist clinic in the province, and at that time, on every Monday, there will be Li Qiu, deputy chief physician and his team to screen, treat and educate people at risk of diabetic foot, as well as to change medication and dispose of foot patients in the clinic. . Through the specialized diabetic podiatry clinic, we can better serve patients by organically linking diabetic foot education, prevention, inpatient treatment and outpatient treatment. We hope that through these efforts, we can drive and guide the development of diabetic foot prevention and treatment in our province.
