Suggested procedure for sequential treatment of congenital cleft lip

1.Prenatal: ultrasound at delivery to determine fetal cleft lip – psychological counseling. 2.First visit: 14 days to 1 month after birth to determine the defect and feeding guidance. If it belongs to incomplete cleft lip, preoperative nasal mold can be worn to promote normal development of nasal cartilage if available. 3.Pre-operative evaluation: 2 months re-visit to assess the development to decide the surgery time and make an appointment for surgery. 4.Surgery time: 3-4 months of age, weight over 5 kg, stage I cleft lip revision. You need to wear a nasal brace after surgery. 5.Review: one month and three months after surgery. Postoperative nasal brace is recommended to be worn for more than six months. After the upper lip suture is completely removed one month after surgery, it must be massaged with medicine. 6.Second-phase revision: For those who are not satisfied with the appearance of the upper lip, it is recommended that the second-phase revision should be done around 2.5 years old. If the nasal wing collapse is obvious, it is recommended to perform the second stage of nasal deformity correction at the same time at the age of 4-4.5 years. 7.Bone grafting for alveolar cleft: review and assess the alveolar cleft defect at the age of 8, and orthodontic treatment before surgery. 8-10 years old, perform bone grafting for alveolar cleft. Continue orthodontic treatment after surgery. 8.Nasal cartilage scaffold implantation: After the completion of the whole body bone development at the age of 16-18, nasal cartilage implantation will be decided according to the shape of the nose.
