The patient, female, 85 years old, farmer, was first diagnosed on September 17, 2013. Lu Yongkun from the Department of Encephalopathy, The First Affiliated Hospital of Henan College of Traditional Chinese Medicine, has been suffering from poor mental health, drowsiness and fatigue for the past 3 years, especially after meals, and has been depressed, drowsy and fatigued for half a month. The diagnosis is that phlegm and dampness trap the spleen, the three jiao are not smooth, the clear yang does not rise, and the clear orifices are not nourished. The treatment is to promote the spleen and promote Yang, resolve dampness and move Qi, and open the orifices to awaken the mind. Prescription: Chen Pi 10 g, Fa Xian Xia 10 g, Fu Ling 25 g, Citrus Aurantium 10 g, Zhu Ru 15 g, Shi Calamus 15 g, Yuan Zhi 12 g, Sha Ren 6 g, Huo Xiang 12 g, Pei Lan 12 g, Ge Gen 30 g, Cang Zhu 15 g, Shen Qu 15 g, Licorice 6 g. 5 doses with water, 1 dose daily, avoid cold and oily things. Second Diagnosis (September 24, 2013): After 7 doses, the patient’s spirit improved, daytime sleepiness was significantly reduced, but he was still weak and drowsy, his head was drowsy, his sleepiness was reduced after lunch, he slept more at night, his mouth was less sticky, he still had less food, his stools were loose, his tongue was light red, his moss was thin and greasy, and his pulse was thin. The patient’s symptoms of phlegm and dampness were reduced, but the loss of spleen health was not reduced. The above formula was reduced by patchouli and pelargonium, and added 15 g of Radix Codonopsis, 15 g of Rhizoma Atractylodis Macrocephalae, and 30 g of fried yam to enhance the power of spleen health. Third diagnosis: (October 01, 2013) daytime drowsiness and sleepiness basically disappeared, dizziness and dullness reduced, spirit turned better, food intake gradually increased, stool forming, tongue red, greasy moss slightly yellow, pulse thin string. Guarding the above to strengthen the spleen, resolve phlegm and dispel dampness, open the orifice and awaken the mind, adding 12g of Radix Bupleurum and 12g of Radix Scutellariae, ascending the clear and descending the turbid, and regulating the Qi flow of the three jiao. Note: “Suwen – angry Tongtian theory”: “Yang Qi, essence is to nourish the spirit, soft is to nourish the tendons.” If Yang Qi does not rise, God loses its nourishment, and thus cannot be shrewd and clever. Danxi Xinfa” cloud: “the spleen and stomach suffer from dampness, sinking sleepy and weak, lazy and sleepy.” It can be seen that dampness trapped in the middle Jiao, clear Yang does not rise, is one of the causes of drowsiness caused by the Wei Qi staying in the Yin part for a long time. In this case, the patient’s spleen and stomach are out of function, phlegm and dampness are trapped in the middle jiao, clear yang cannot rise, phlegm and qi are depressed, and the three jiao are out of harmony, resulting in sleepiness and drowsiness. The treatment should be to strengthen the spleen, resolve dampness, awaken the mind and open the orifices, and at the same time to smooth the qi flow of the three jiao. Radix Bupleurum elevates Yang energy, dredges the liver and relieves depression, while Scutellaria baicalensis drains heat by bitter cold; pungent, open and bitter, elevates the clear and lowers turbidity, and strains and clears the three jiao qi channels.
