What is the treatment after hand trauma

In our clinical work, we have noticed a group of patients with a clear history of trauma or surgery on the back of the wrist, the back of the hand or the dorsal proximal part of the fingers. The fingers of this hand have stagnant nail growth, showing symptoms of onychomycosis, some patients with a small amount of yellowish clear liquid overflow between the nail epithelium and the root of the nail, and the rest of the patient has no other symptoms and discomfort; generally, it is more common in the 2-4 fingers, and less common in the thumb. For this kind of patients, we give iodophor disinfection or alcohol gauze wet compresses every day, keep the affected area dry, avoid contact with sewage, observe the changes of the condition, after one month without significant improvement, that is, under local anesthesia, nail extraction surgical treatment. Postoperative epithelial skin swelling gradually subside within 2 weeks, follow-up six months all patients epithelial skin swelling without recurrence, normal growth of nails. This phenomenon has many sayings, I think: normal nail growth, is the old nail in the nail bed, relying on the newborn nail constantly grow to advance, forward growth. After the trauma, all kinds of exogenous reasons, so that the original nail bed and nail closure, no longer forward gliding growth. Instead, the new nail formed by the nail mother cells grows immediately above and along the nail bed toward the fingertip. The combination of old and new nails causes skin bulges and localized infections. Treatment: When conservative treatment fails, the nail should be extracted in a timely manner. The new nail is allowed to grow in an unobstructed environment.
