2. Information and test data entry

3. HLA prime-match remobilization

4. Retain blood samples again for high-resolution matching compound testing

Professionals will need to take another blood sample from the volunteer donor and send it to a designated laboratory for HLA high-resolution compatibility testing. When the test determines that the HLA of the volunteer donor is compatible with the HLA of the patient requiring HSCT and meets the requirements for transplantation, the donor signs a consent form and agreement to donate, and a pre-collection physical examination is arranged by the repository management center and its affiliates at the designated hospital.

5. Pre-collection physical examination

The volunteer hematopoietic stem cell donor is evaluated by a specialist on the results of the physical examination and the physical and mental condition of the volunteer donor to confirm that the volunteer donor is eligible for hematopoietic stem cell donation, and the Chinese Bone Marrow Bank Management Center informs the above overall situation to the hospital that applied for a match for the volunteer donor and the patient, and then the transplant hospital, the database and the collection hospital work together to develop the collection and transplantation plan.

6. Injection of hematopoietic stem cell mobilizer

After the collection and transplantation plan is developed, the transplantation hospital, the repository, and the collection hospital proceed with simultaneous collection and transplantation preparation based on the plan, respectively. The proposed peripheral blood HSC donor needs to receive subcutaneous injection of HSC mobilizer (also called inducer, granulocyte colony-stimulating factor, G-CSF) at the collection hospital or collection center 4 to 5 days before collection to induce a large number of HSC from the red bone marrow into the peripheral circulating blood and to be monitored.

7. Collection of peripheral blood hematopoietic stem cells

Hematopoietic stem cell collection can be initiated when the density of hematopoietic stem cells in peripheral blood, has reached an ideal state, usually on the fifth day of hematopoietic stem cell mobilizer injection. The process of collecting HSCs is the same as that of blood donation, where a machine separates the blood flowing in the blood vessels and extracts the peripheral blood HSCs from it. The whole process takes about 3 to 5 hours, and the amount collected depends on the weight of the patient and donor and the amount of HSCs needed for transplantation.

If the patient needs a small number of HSCs and the donor is heavy, one HSC collection is sufficient; if the number of HSCs collected at one time does not meet the need, another collection will be made the next day, with approximately 200 mL of HSC suspension collected each time. Each therapeutic amount of HSC suspension usually contains 5 to 10 grams of HSCs.

8. Post-collection observation

After the needle is removed at the end of the collection, continue to observe and rest as requested by the physician, and after 2 hours without abnormal symptoms, you can perform soothing activities around the blood collection room.

9. Follow-up

Retrospective physical examinations and follow-up visits will be performed according to the physician’s requirements and protocols.
