Abstract: The patient in this case is a pregnant woman at 25 weeks’ gestation who presented with intermittent chest tightness and palpitations 2 days ago. She was admitted to the hospital for electrocardiogram, cardiac triplex, and cardiac ultrasound, and was diagnosed with myocarditis, and was treated with medication after admission, and her condition improved after 1 week of hospitalization.
Basic information】Female, 31 years old
Disease Type】Myocarditis
Hospital】The First Affiliated Hospital of Xi’an Jiaotong University
Time of consultation】December 2018
Treatment plan】Medication (Astragalus granules, vitamin C tablets, coenzyme Q10 tablets)
Treatment period】4 weeks of inpatient treatment, 6 months of outpatient follow-up
【Treatment effect】Recovery of myocarditis, normal fetal development
I. Initial consultation
The patient, female, 31 years old, had paroxysmal chest tightness, palpitations and posterior back discomfort with no obvious cause 2 days ago, which lasted for more than 10 hours and relieved. The blood test showed elevated leukocyte and neutrophil ratios, elevated troponin and creatine kinase in the three cardiac tests (immuno-quantitative), and a cardiac ultrasound that was consistent with the sonogram of myocarditis, so the patient was initially diagnosed as having myocarditis and was admitted to the hospital for treatment.
II. Treatment history
After admission, the examination showed that the fetal heartbeat and fetal movement were normal at 25 weeks of gestation, and the viral serological test indicated the presence of viral infection. Therefore, in order to suppress and kill the virus to ensure the health of mother and child, I communicated with the patient and her family and explained to them that the correct use of medication during pregnancy would not cause harm to mother and child, but if treatment was delayed, it would increase the chance of fetal malformation and late pregnancy infection. The family and the patient agreed to take medication after consideration. Astragalus granules were given for routine antiviral treatment, and oral vitamin C tablets and coenzyme Q10 tablets were administered to improve the symptoms, and continuous fetal heart monitoring was performed during the medication period.
III. Treatment effect
After 1 week of in-hospital drug treatment, the patient’s viral serology test, troponin and creatine kinase isoenzyme results decreased after rechecking, indicating that the drug treatment was effective, and the patient had no complications during the treatment, and the fetal heart monitoring continued to be normal, and the ultrasound indicated good fetal development without abnormalities. After discharge, the patient was told to continue taking Astragalus granules, vitamin C tablets and coenzyme Q10 tablets, and to go to the hospital for review after 3 weeks and stop taking them as prescribed by the doctor. The patient’s ECG, myocardial triad and cardiac ultrasound were all normal, indicating that the myocarditis was cured.
IV. Notes
The patient was obviously relieved to see the results of the follow-up examination, and I was happy for the patient’s recovery. I also told the patient that her previously diagnosed myocarditis was related to viral infection, so she should pay attention to daily protection, add clothes in time, and do exercises such as walking and yoga during pregnancy accompanied by her family, which have positive effects on maintaining her resistance. In addition, if there is no other discomfort, such as high blood sugar, you can eat more fresh fruits to supplement vitamin C, which can also strengthen the body resistance.
V. Personal insight
The patient in this article is a viral infection, resulting in myocarditis, mostly after 1-3 weeks of infection will appear chest tightness, palpitations and other uncomfortable symptoms, and because there is no special drug, so only antiviral and symptomatic treatment treatment, once the delay in treatment will not only increase the difficulty of treatment, but also may threaten the health of the mother and child, fortunately, the patient in this article, the treatment is more timely, and their own physical quality is high, so Fortunately, the patient in this article was treated in a timely manner and had a high level of health, so there were no serious consequences.
