Treatment of herniated zygomatic arch

  The oval-shaped facial profile is commonly known as “melon face” or “goose egg face” and is recognized as the ideal face shape for oriental women. The oval-shaped face is well-proportioned, well-proportioned, well-defined, and in line with the beauty of the curve. However, when women have prominent cheekbones, it visually gives them a masculine face and makes them look “unlucky” and rigid. The aesthetic requirements of the East Asian population are to reduce the height of the cheekbones.  Since 1980, many domestic and international surgeons have been performing zygomatic arch reduction through different surgical methods. Previous surgical methods include: zygomatic arch grinding, I-shaped/L-shaped zygomatic arch debridement, etc. Zygomatic arch grinding is suitable for patients with mild zygomatic arch protrusion, but after the surgery, adverse effects such as osteophytes and left-right asymmetry can occur in the long term.  I-shaped/ L-shaped zygomatic arch debridement is suitable for moderate to severe zygomatic prominence, but it requires a large area of attached soft tissue to be peeled off, which may cause intraoperative nerve damage, postoperative facial depression and long-term skin sagging.  Currently, a safer and more effective surgical procedure is the Intraoral and Preauricular Wedge-Section Osteotomy to achieve zygomatic arch reduction. The clever use of intraoral incision, no obvious postoperative scars, no need for extensive peeling of attached tissues during surgery, and less likely to cause long-term postoperative facial sagging and other undesirable side effects.
