One-needle therapy with tui na for vascular migraine

  Vascular migraine is one of the common acute headaches and is a group of periodic attacks with a family tendency to develop. It is a periodic disorder caused by episodic vasodilator dysfunction and temporary abnormal secretion of certain body fluids (such as norepinephrine, bradykinin, prostaglandin, etc.). The clinical manifestations are: hallucinations and hallucinations, partial blindness and other transient brain dysfunction before the attack, followed by a one-sided headache, which is paroxysmal and may present stabbing, swelling, dull, throbbing drilling and tearing pains, often lasting for hours or days. It usually recurs at intervals of several weeks and is cyclic in nature. This treatment was applied to 36 patients in 2 years from June 2008 to June 2010, and achieved remarkable efficacy, which is reported as follows: 1. There were 20 male cases and 16 female cases, aged 24-58 years, with the duration of the disease ranging from 2 weeks to 7 years, and the pain within 6 months accounted for 70%, 19 cases on the left side of the disease and 17 cases on the right side, of which 18 cases had more than 4 attacks per month, and 24 cases had headache for more than 48 hours per attack. 36 patients mostly received medication, acupuncture and massage treatment, but the efficacy was poor. Most of the headaches had symptoms such as nausea, vomiting, vertigo and photophobia of varying severity.  2. Treatment 2.1 Acupuncture: Take the Tai Chong point on the healthy side for migraine on the affected side, i.e., take the left side for right disease and the right side for left disease. Take Taichong point on the healthy side, after routine sterilization, use 2-inch stainless steel milli-needle to pierce 0.8-1.5 inches at an angle of 45 degrees obliquely toward the heel, perform lifting and inserting, twisting and turning the diarrhea method, so that the patient has acidity, swelling, numbness and numbness, and uploading by the meridian is better, do not use strong stimulation technique, so as not to induce headache aggravation by patients’ fear of pain and tension. Every 5-10 minutes, the needles are performed once, and the needles are kept for 30 minutes, once a day, 7 times for a course of treatment. The efficacy should be evaluated after 1-2 courses of treatment.  2.2 Tui Na: 2.21. Ring pushing method: the patient sits, the doctor takes the line from the affected side of the silk bamboo hollow to the rate of the valley, do ring pushing for 3-5 minutes, frequency 60-90 times / minute, the force is steady and gentle and gentle, the action is continuous and even. This method can dredge the meridians, improve the blood circulation in the head.  2.23. Push the sun: push the affected side of the temple with the radial side of the thumb from the front to the back to the edge of the hairline, apply for 3-5 minutes, frequency 60-90 times/minute. This method can awaken the brain and brighten the eyes, and relieve wind and pain.  2.24. Sweeping method: The patient is seated, the doctor holds his head with one hand, the other hand thumb interphalangeal joint radial side or thumb radial side from the patient’s head dimension, along the hairline to the direction behind the ear to do rapid back and forth pushing and rubbing, the remaining four fingers slightly curved to help, along with the thumb movement, while doing pushing and rubbing action for 3-5 minutes, frequency 60-90 times / minute, gentle and gentle force. This method has the effect of pacifying the liver and submerging the yang, activating the ligament and relieving pain.  3. Efficacy criteria and treatment results Efficacy criteria: cure: all the patients’ self-conscious symptoms and positive signs disappeared after 2 courses of treatment, and the headache did not recur after 6 months of follow-up; effective: the pain was significantly reduced, and the pain interval was significantly extended, less than 1 month, more than 6 months; invalid: the pain was reduced, and the interval did not change significantly.  Treatment results: 36 patients were cured in 29 cases, improved in 6 cases and invalid in 1 case, with an efficiency of 97.22%.  4. Typical case Liu, female, 34 years old, a teacher, visited the clinic on December 27, 2008. The patient had been suffering from left-sided headache for 6 months, with frequent attacks for 2-3 days per week, lasting for several hours each time, with bruises around the left temple, often affecting the ipsilateral eye, or blurred vision and restlessness. Examination: the patient had painful expression, red face, red tongue, thin yellow moss, sunken string pulse; he felt headache and dizziness, irritability, tinnitus and bitterness in the mouth, fatigue, diagnosed as vascular migraine. After acupuncture, the pain was greatly reduced after one treatment, and after 10 times of treatment, all symptoms and positive signs disappeared, and there was no recurrence after one year of follow-up.  5. Experience: This disease is classified as “migraine” and “migraine head wind” in Chinese medicine. It is believed that this disease is closely related to the liver, spleen and kidney. If it is caused by the liver, it may be caused by liver yin deficiency or liver yang hyperactivity; or liver qi stagnation, long depression and fire, which disturbs the clear orifices and causes pain. If it is due to the spleen, or if the spleen is deficient in biochemical power, the qi and blood are deficient. If the headache is due to the kidney, it is caused by overworking and depletion of the kidney essence, resulting in the emptiness of the medulla; or if the kidney yang is weak, the cold is generated from within and the clear yang is not open; or if the kidney yin is insufficient, the water does not contain the wood and the wind yang is hyperactive, resulting in headache. If the headache does not heal for a long time and the pain is like a cone or a thorn, it is due to the long-standing disease entering the luo and blood stasis and luo paralysis. In female patients, migraine attacks are often related to menstruation, mostly around the time of menstruation, and the attacks can be reduced after pregnancy. In addition, it is also related to fatigue, mental tension, emotional excitement, lack of sleep, and external attack of the six evil spirits.  In recent years, with the accelerated pace of life, people’s life has become more and more stressful, so the incidence of this disease has a tendency to increase. According to the author’s clinical observation for many years, this disease is mostly caused by liver qi stagnation, qi stagnation turning into fire, liver fire inflammation resulting in head swelling and pain or dizziness, migraine is mainly in the lateral part of the head, which should be the main treatment area of the foot Shaoyang bile meridian, but the foot Shaoyang bile meridian and the foot Jurgyin liver meridian are cognate, and the Tai Chong point is the infusion point and the original point of the foot Jurgyin liver meridian. The original point is where the vital energy of the internal organs is infused, passes through and stops. The vital energy of the internal organs is rooted in the kidney, and the vital energy runs throughout the body through the Sanjiao, which is the root of the 12 meridians. The headache will be relieved. As for taking the Jian side of Taichong instead of the affected side, it is found from years of clinical experience that the pressure pain on the Jian side of Taichong is stronger than the affected side of Taichong, so the affected side is taken instead of the Jian side. The acupuncture of the Tai Chong point is simple and practical, and it can greatly reduce the patient’s pain, which is complementary to the above-mentioned acupuncture techniques.
