What are the Chinese medicines for cholecystitis?
Cholecystitis is a relatively common disease with a high incidence. According to its clinical manifestations and clinical course, it can be divided into two types: acute and chronic, often combined with cholelithiasis. Acute cholecystitis with severe pain or colic in the right upper abdomen, mostly caused by stones or parasites embedded in the neck of the gallbladder, is often sudden and very intense, or presents a colic-like pain. In non-obstructive acute cholecystitis of the gallbladder duct, the pain in the right upper abdomen is usually not severe, but mostly persistent and distending.
Clinical manifestations
1. Acute cholecystitis
The clinical manifestations of acute calculous cholecystitis are basically the same as those of acute non-calculous cholecystitis.
(1) Symptoms
The pain is often sudden, very intense, or colic-like, occurring mostly after eating high-fat food, mostly at night; the general pain in the right upper abdomen is seen in non-obstructive acute cholecystitis of the gallbladder duct, and the pain in the right upper abdomen is generally not intense, but mostly persistent and distending, and as the inflammation of the gallbladder progresses. The most common radiating sites are the right shoulder and the subscapular angle of the right scapula, which are caused by the inflammation of the gallbladder stimulating the right phrenic nerve endings and the peripheral nerves of the abdominal wall.
Nausea and vomiting are the most common symptoms. If nausea and vomiting are stubborn or frequent, they can cause dehydration, deficiency and electrolyte disturbance, mostly seen when stones or roundworms obstruct the gallbladder duct.
③ Chills, chills and fever Mild cases often have chills and low fever; heavy cases may have chills and high fever, with fever up to 39℃ or more, and psychiatric symptoms such as delirium and delirium may occur.
(4) Jaundice Rarely, jaundice, if any, is usually mild, indicating that the infection has spread to the liver via the lymphatics, causing liver damage, or that the inflammation has invaded the common bile duct.
(2) Main signs The abdominal examination shows tension of the right upper abdomen and middle epigastric abdominal muscles, pressure pain, rebound pain, and positive Murphy’s sign. In cases with pus accumulation in the gallbladder or peribiliary abscess, a mass with pressure pain or an obviously enlarged gallbladder can be found in the right upper abdomen. When abdominal pain and abdominal muscle tension extend to other regions of the abdomen or the whole abdomen, it suggests gallbladder perforation. Or there may be acute peritonitis. Mild jaundice may occur in 15% to 20% of patients due to peripheral edema of the gallbladder duct, gallstone compression and liver damage caused by pericholangitis, or inflammation involving the common bile duct, causing spasm and edema of the sphincter of Oddi and resulting impaired bile drainage. If the jaundice increases significantly, it indicates the possibility of obstruction of the common bile duct with stones or complications of choledocholithiasis. Severe cases may show signs of peripheral circulatory collapse. Blood pressure is often low, and infectious shock may even occur, especially in severe cases of septic gangrene.
2.Chronic cholecystitis
(1) symptoms persistent dull pain or discomfort in the right upper abdomen; indigestion symptoms such as nausea, belching, acid reflux, abdominal distension and heartburn; pain in the right lower scapular region; worsening of symptoms after eating high-fat or fatty foods; long duration of the disease, alternating acute attacks and remission; acute attacks are the same as acute cholecystitis symptoms; remission can sometimes be asymptomatic.
(2) Signs There may be mild pressure pain and percussion pain in the gallbladder area, but no rebound pain; in cases of cholestasis, a distended gallbladder may be found; there may be muscle tension in the right upper abdomen during acute attacks, and the body temperature may be normal or hypothermic, and jaundice may occasionally appear. The pressure point of gallbladder is at the intersection of the right rectus abdominis muscle outer edge and the rib arch, the pressure point of thoracic spine is beside the 8-10 thoracic vertebrae, and the pressure point of right phrenic nerve is between the two lower corners of the right sternocleidomastoid muscle in the neck.
