Can a new type of pneumonia be severe without significant fever?

Severe and critical cases of novel coronavirus pneumonia are likely to present with no fever or only a low-grade fever, unlike other respiratory infections. As with influenza, the higher the patient’s temperature, the greater the likelihood of pneumonia. The temperature may peak when symptoms are most severe. However, in the case observation of novel coronavirus pneumonia, regardless of the normal, severe, or critical type, there may be only low or moderate fever. Some cases of severe and critical types may have no fever. Therefore, the likelihood of a severe case should not be determined by the presence or absence of fever or high or low temperature; attention should be paid to chest CT and respiratory manifestations. The presence of shortness of breath, >30 breaths per minute in adults, or a 50% increase in the extent of lung lesions within 48 hours on CT suggests a high likelihood of severe cases of novel coronavirus.
