Observation on clinical efficacy of laser integrated method in treating hemangioma

Pulsed dye laser (SPTL) wavelength 585nm, pulse width 50ms, pulse energy density 3~10J/cm2; Nd: YAG laser, wavelength 10645nm, output power 0~100W, continuously adjustable, the fiber core diameter of 0.5mm, spot diameter of 2mm; pulsed CO2 laser, wavelength 10.6um, output power 0~20W, the following five methods of treatment: 1, SPTL laser + prednisone (treatment group 1) eye, face, ear, vulva, labia rapid growth of capillary hemangiomas and strawberry hemangiomas. Prednisone is a kind of glucocorticoid, which can inhibit cell mitosis and reduce vascular endothelial proliferation at the stage of lesion development, thus slowing down the growth of hemangioma and promoting tumor regression. 585 nmSPTL laser is at the peak of hemoglobin absorption, and the thermal effect produced by the lesion tissues after absorbing the laser energy will make the blood vessels contract and coagulate, and then gradually fade away due to lack of oxygen, because it is a selective photothermal effect without causing any damage to the blood vessels. Selective photothermal effect does not cause epidermal damage, and it has special effect on lesions with small and superficial vascular diameter”. There is a synergistic effect between the two combined treatments. From the clinical observation, this group basically controls the tumor development in 2~3 months of treatment and lifts the risk factors. 2.Pingyangmycin+Nd:YAG laser treatment is mainly used for oral hemangioma, the oral cavity has special function, so the treatment requires high efficacy, minimize the impact on oral function and take into account the effect of cosmetic surgery. The surgical wound is large, the wound is affected after suture and bandage, the 585nm SPTL laser penetrates shallowly and is not well absorbed by the mucous membrane containing much moisture, so it is difficult to achieve the curative effect. Therefore, intratumoral injection of paxillin is used to inhibit the synthesis of cellular DNA and cut off the DNA strand, so as to achieve the inhibition of the endothelial cell proliferation and promote the degradation of the hemangioma. Selection of Nd: YAG laser action on hemangioma after blood flow slowed down, blood viscosity increased, resulting in local acidosis and make the vascular endothelium degeneration, tissue hypoxic necrosis. It improves the total effective rate of treating hemangioma and reduces the side effects of laser and pingyangmycin accordingly. 3.SPTL laser + tretinoin treatment of hemangioma lesions does not cause harm to vital organs, but due to the lesions occurring in the fingers, and toes, palms, joints, perineum and other special parts of the dermis, the treatment is not easy to receive a good effect, and the treatment is not good enough to produce a scar or produce dysfunction. Trimethoprim is a hormone injection, the principle of action is the same as prednisone, local injection can reach the depth of the lesion, so that the thick and deep blood vessels atrophy, and no oral side effects. SPTL laser has better effect on surface lesion treatment, and the combination of the two can achieve perfect treatment of deep and shallow layers, which has good efficacy and can reduce side effects. 4.SPTL laser + CO: laser treatment of nevus erythematosus hypertrophic nodule, SPTL laser treatment will not leave scar, but clinical observation of its lesions on the nodule is difficult to work. Therefore, when a large area of nevus erythematosus lesions with SPTL laser treatment nodules, but more prominent, can not achieve the goal of cosmetic treatment.
