For strawberry hemangioma in the proliferative stage within 1 year old, early medication can be used to control the further development of the hemangioma and promote its remission. Clinically available drugs include 5% imiquimod cream, 0.5% timolol maleate eye drops, propranolol oil or emulsion, etc. Timolol maleate is a non-selective beta-adrenergic receptor blocker with similar and stronger effects than propranolol. Ni et al. first reported the successful treatment of a 4-month-old infant with episcleral hemangioma with timolol maleate eye drops. chakkittakandiyil et al. treated 73 children with timolol maleate eye drops for a mean of 3.4 ± 2.7 months. Oranje et al. reported that in 20 children treated with topical timolol maleate eye drops (1-4 months), except for 3 cases with no improvement in deep hemangioma, the lesions began to slowly regress after growth control in the remaining 7 children. In China, some authors compared the efficacy and safety of two topical drugs, imiquimod cream and timolol maleate eye drops, on superficial hemangioma of infants and children in the proliferative stage, and the imiquimod cream group generally began to take effect 1 to 3 weeks after topical application, with an average onset of effect of 14 days and a treatment efficiency of 81.4%. In the group of timolol maleate eye drops, the onset of action was from 3 d to 3 weeks after topical application, with an average onset of action of 8 days and a therapeutic efficiency of 88.9%. After topical application of imiquimod cream, skin reactions may occur to varying degrees, including erythema, peeling, breakout, crusting and superficial scarring. It is believed that timolol maleate has a relatively rapid onset of action, promotes a higher degree of lesion color fading and has fewer adverse effects, and can be the topical drug of choice for the treatment of superficial hemangiomas in the proliferative phase. In recent years, I have received good clinical results by using this method or combining it with other methods of comprehensive treatment.
