131I thyroid clearance treatment dose

1. 30mCi is recommended for thyroid clearance in intermediate- and low-risk patients.
The recommended adjuvant dose of 131I is 150 mCi for intermediate- and high-risk patients with suspected or proven microscopic residual lesions or highly invasive histologic subtypes (hypercellular, columnar cell, etc.) without distant metastases. 3. Consider higher doses of 131I for patients with large amounts of residual thyroid tissue or focal clearance after incomplete/near-total thyroidectomy.
For patients with residual surgically unresected DTC tissue in the neck, with inoperable or refractory cervical lymph nodes or distant metastases, or with unexplained elevated serum Tg levels after total thyroidectomy, especially irritant Tg levels, thyroid clearance should be combined with focal clearance therapy at a 131I dose of 100-200 mCi.
