A 50-year-old male with diffuse hepatocellular carcinoma of the left liver and left caudal lobe (throughout segments I, II, III, and IV of the liver), measuring approximately 17x300px, invading the left hepatic vein, middle hepatic vein, with a left branch of the portal vein with cancer thrombus, and left retroperitoneal lymph node metastasis. Left hemicolectomy, left caudal lobe resection, portal vein cancer thrombus removal, retroperitoneal lymph node dissection and cholecystectomy were performed. The tumor was completely resected, the remaining liver surface was clean and neat, the cancer thrombus was completely removed, and the patient recovered well.
