Treatment of gallbladder polyps

  The treatment of gallbladder polyps mainly involves the determination of the benignity and malignancy of gallbladder polyp-like lesions, so as to achieve early detection of malignant lesions and precancerous lesions and early surgical removal.  (1) The size of gallbladder polyps: Most scholars have concluded that the size of gallbladder polyps is related to their benignity and malignancy. Small gallbladder polyps (diameter <10mm) have been found to be mostly benign lesions and can remain unchanged for many years, which can be observed and regularly reviewed by ultrasound. The incidence of cancer in gallbladder polyps larger than 10mm is significantly higher than that in polyps smaller than 10mm, which should be further examined or closely followed, and operated as soon as the change; (2) the number and shape of polyps: single, low base wide polyps are mostly considered to be true polyps, that is, adenomatous polyps, which are sometimes difficult to determine in ultrasound examination, which should be dynamically observed, and if the growth rate is rapid, should Further enhanced CT or angiography ultrasound should be done to see if the polyps are rich in blood flow.  (3) Combined gallbladder stones: the relationship between gallbladder cancer and gallbladder stones is clear, and some patients with gallbladder cancer can be combined with gallbladder stones. Therefore, the risk of gallbladder cancer is increased by the presence of stones. Also, if gallbladder stones develop clinical symptoms, they need to be removed together with gallbladder polyps.  (4) Accompanying clinical symptoms: Some patients with gallbladder polyps have right upper abdominal pain and back pain, especially when the polyps are located near the neck of the gallbladder and the opening of the cystic duct, the polyps can easily block the cystic duct and cause obstruction of bile drainage, for which surgery is also needed.  Although there are some controversies in the surgical management of gallbladder polyp-like lesions, in general, for gallbladder polyps with diameter <10mm and no clinical symptoms, ultrasound examination can be performed regularly, while for gallbladder polyps with diameter >10mm, single, broad-based and combined gallbladder stones with clinical symptoms, surgery is recommended.
