In our center, we promote individualized treatment, including the “microstimulation” protocol, the advantages of which are strongly promoted by those who have obtained good quality embryos or successful pregnancies, while on the other hand, the “microstimulation” is viewed with suspicion. In fact, whether it is a myth or an underestimation of the effectiveness of microstimulation, it is due to an incomplete understanding of it. In order to stop the confusion, we will introduce you to the “ovarian microstimulation” program from the following aspects. I. What is ovarian microstimulation protocol “Microstimulation” means that the stimulating effect on the ovaries is relatively weak. In contrast to conventional IVF treatment protocols, patients use only a small amount of medication to obtain a small but high quality number of eggs. How is the microstimulation protocol created? Compared to conventional protocols, microstimulation yields fewer eggs and potentially fewer embryos, but why would such a protocol even exist? The first IVF baby was born in 1978 and she was derived from a single follicle of a natural cycle. Subsequently, as research on ovulation-promoting drugs progressed, different ovulation-promoting protocols were gradually developed, which were widely used for their ability to produce more eggs and embryos in a single treatment cycle and to improve success rates. After more than 30 years of clinical experience, experts are demanding a new approach to these non-physiological hyper-ovulatory treatments: safer, more efficient, more comfortable and more simplified, which implies the need to find a treatment strategy that combines the advantages of the various protocols in a natural cycle and in a conventional protocol. Rather than being a banner of change in ovulation protocols, I prefer to think of microstimulation as a return to scientific introspection. At the same time, microstimulation is a product of its time. With the development of society, the average age of infertility patients is gradually increasing, and it is inevitable that the ovarian function of older women is declining, even with super large amounts of medication, only a small number of follicles grow and the quality of eggs is not ideal. What are the advantages of the microstimulation protocol? The treatment process is simple and the number of follow-up visits is reduced; the egg retrieval procedure is simple and the pain and discomfort are reduced; the ovaries are less disturbed due to over-stimulation and the eggs are closer to their natural state and the quality of the eggs is better; less medication, less cost and less painful injections are used; the risk is low; the miscarriage is low and the baby is well. What is the treatment cost of the microstimulation program? Due to less medication, patients spend much less on medication, the total cost per treatment cycle needs to be about 15-20,000, the cost effectiveness of a single cycle is increased. Some patients with reduced ovarian function may need several cycles in a row to accumulate embryos, therefore, the total cost of treatment still needs to be budgeted and prepared in advance. V. Why is the microstimulation protocol not the first choice for all patients? We will choose the most suitable, successful and safe option for each patient based on age, ovarian function and previous response to ovulation medications. Individualized treatment is necessary. Microstimulation is a direction and a trend in ovulation promotion and its rational clinical application will definitely lead to more patients’ success.
