Many patients who come to the clinic say that they have to get up every morning to go to the toilet, which sounds like a good habit, but it is a necessity. Every morning, they get up at 4:00 or 5:00 to go to the toilet, and their stools are still unformed, or even pull out undigested food, especially when winter comes, this situation is more serious, and it takes courage to leave the warm blanket in winter, which is very painful in the long run. In fact, this condition is called wu shifting diarrhea in Chinese medicine, named according to the time of diarrhea, 3-5 am in the twelve hours is the time of Yin, which is also the time of the fifth shift. It is also known as kidney diarrhea, which is named according to the etiology of the disease. The diarrhea is caused by the deficiency of spleen and kidney yang and the failure of the vital fire. The general manifestations are intestinal tinnitus and diarrhea, umbilical pain, pain reduction after diarrhea, thin stools, mixed with non-consuming food, accompanied by symptoms such as cold form and cold extremities, unheated extremities, cold waist and knees, fatigue and weakness, clear and long urine, frequent night urination. At the time of the fifth watch, the sky is about to dawn, which is also called “Ping Dan” in Chinese medicine, Yang Qi starts to rise and warm the whole body, but the spleen and kidney Yang is deficient, the vital gate fire decreases, Yin cold prevails inside, Yang Qi is strangled, Yang Qi is not fixed, the warming loses its source, and diarrhea develops. The diarrhea is usually more serious in autumn and winter than in spring and summer, because autumn and winter, especially in winter, are mainly for astringent Yang Qi, and the cold climate damages Yang Qi more, so we must pay attention to warmth and active treatment in winter, otherwise it will further aggravate the disease. In Chinese medicine, there is a special medicine for this type of diarrhea – Sishenwan. It is mainly used for diarrhea caused by deficiency of kidney yang, bloating of the intestines, loose diarrhea of the fifth shift, and failure to transform the finished grain. Nutmeg is pungent and warm, enters the spleen, stomach and large intestine meridian, warms the middle and astringent intestines, moves qi and eliminates food. Sometimes Wu-shan diarrhea is not so typical, but nowadays various cold drinks and air conditioners damage more yang, and there are many people with chronic diarrhea from spleen and kidney yang deficiency. Clinically, Sishenwan is not only used for “five-shift diarrhea”, but also for patients with deficiency of cold in the spleen and stomach and kidney yang.
