Understanding High Uric Acid and Gout – A Medication Guide

Gout is known as “the disease of the emperor” and “the disease of the rich”, and its incidence is increasing year by year in modern society where the standard of living is gradually improving. Therefore, it is crucial to control and manage diet while applying drugs to relieve acute pain. With hyperuricemia, which is a precursor to gout, should we take medication or not? When to start treatment? What should I pay attention to during treatment? For some people with hyperuricemia, it is true that they do not need to take uric acid-lowering drugs. There are three indicators to determine this: 1) high or low uric acid value; 2) absence of “symptoms”; “absence of symptoms” does not mean “no pain” as people think, but has two meanings: no gout (acute arthritis); no gout stones, kidney stones or kidney insufficiency, etc. 3. The presence of “cardiovascular risk factors”; to determine whether there are “cardiovascular risk factors”, a doctor must assess them in person, such as hypertension and diabetes are all risk factors. If there are no symptoms and the uric acid value is < 480 μmol/L, there is generally no need to take medication. If there are no symptoms, no cardiovascular risk factors, and the uric acid value is between 480 and 540 μmol/L, medication is generally not required. If you also have cardiovascular risk factors such as hypertension and diabetes, and your blood uric acid is > 480 μmol/L, you should consider taking medication to lower uric acid. If the physician’s assessment is “no cardiovascular risk factors”, you should start taking medication to lower uric acid when your blood uric acid is > 540 μmol/L. Many patients stop taking acid-lowering medication when their uric acid drops to the target value, but the uric acid rises again soon afterwards and gout attacks start again. When the uric acid has dropped to the target value of 300 – 360umol/L, the correct approach should be to gradually reduce the dosage of medication under the supervision of the doctor; if the uric acid is still maintained at this time, you can continue to reduce the dosage of medication. When the uric acid can be controlled with the least amount of medication, only then can we consider stopping the medication. It must be emphasized that uric acid-lowering treatment is continuous and long-term, even lifelong for patients with chronic gout stone gout, so try to avoid stopping medication without doctor’s supervision. That practice of not taking medication without pain, trying not to take medication, and stopping medication when it is lowered is not right. In addition, it should be pointed out that with gout do not go to buy the so-called Hong Kong and Macao gout spirit and rheumatism spirit, Japanese gout medicine, which often contains hormones, if not under the guidance of doctors long-term use will bring some adverse consequences, such as diabetes, necrosis of the femoral head, which will not be worth the loss. We recommend the combination of Chinese and Western medicine, especially the application of Chinese herbal medicine to clear heat and dampness, activate blood circulation and relieve pain, which can not only promote the excretion of uric acid, but also improve the symptoms of gouty arthritis. In addition, according to our clinical experience, Chinese medicine can improve the “three highs” value of some patients and reduce the risk of cardiovascular disease while treating hyperuricemia. Gout can be cured, the key is to combine prevention and treatment, standardized, long-term. Therefore, we should have confidence and patience, and we will definitely overcome hyperuricemia and gout!
