Gout is not just for the joints

  According to the latest statistics, there are more than 180 million people with hyperuricemia in China, and hyperuricemia has become the “fourth highest” after hypertension, hyperlipidemia and hyperglycemia. Chinese scholars have designated April 20 every year as “High Uric Acid Day”. The purpose is to make people pay attention to the diagnostic standard of hyperuricemia (>420μmol/L), and to make it more popular.  The epidemiology of hyperuricemia and its risks in China Hyperuricemia (HUA) is a disease caused by disorders of purine metabolism that result in blood uric acid concentrations exceeding normal values. In recent years, with the improvement of living standard, change of lifestyle and diet structure, especially the increase of protein- and purine-rich food intake, the number of HUA patients in China has increased steeply, and the incidence of HUA is gradually becoming younger. The prevalence of HUA is higher than the prevalence of diabetes in China, and HUA is characterized by a preference for men over women, with the prevalence of men being significantly higher than that of women.  Since patients with hyperuricemia may not have clinical symptoms in the early stage, they often do not care about the change of blood uric acid value, and thus ignore the early treatment. Gout is only the tip of the iceberg of HUA. Asymptomatic HUA other than gout has no significant clinical symptoms, but its harm is potential and continuous, and the long-term harm is even greater.  It has been proved that HUA is related to many diseases and is as dangerous as hypertension and diabetes to the health of middle-aged and elderly people, which is a problem that cannot be ignored. More and more studies have shown that HUA is closely related to hypertension, left ventricular hypertrophy, insulin resistance, obesity, hyperlipidemia, diabetes, etc. HUA is an important factor that aggravates atherosclerosis and promotes the occurrence and progress of cardiovascular diseases, and also induces and aggravates kidney damage. In addition, HUA can increase the risk of total death, all-cause cardiovascular disease death and cardiovascular events in hypertensive patients.  Emphasis on the management of hyperuricemia The danger of hyperuricemia should be taken seriously and reasonable and standardized treatment should be given early. Patients with hyperuricemia are advised to improve their lifestyle, including a healthy diet, smoking cessation, consistent exercise and weight control, and to avoid medications that elevate blood uric acid.  Since 80% of blood uric acid is produced in the body and only 20% comes from the diet, even if the diet is very strictly controlled, it can only reduce blood uric acid by 70-90 μmol/L, and excessive diet control is harmful to health. Therefore, for patients with blood uric acid >480μmol/L (with cardiovascular and other risk factors) and patients with blood uric acid >540μmol/L (without cardiovascular and other risk factors), it is recommended to use uric acid-lowering drugs to lower uric acid, so that the blood uric acid can be controlled <360μmol/L in a long-term stable manner. L to prevent recurrence and reduce the occurrence of complications.  Since "420" is the diagnostic standard for HUA in men - 420μmol/L, this date also makes it easier for everyone to remember the diagnostic standard for HUA. It is expected that "High Uric Acid Day" will raise awareness of the dangers of the fourth highest HUA, just like World Diabetes Day, World Stroke Day and World Kidney Disease Day.
