Management of thyroid nodules in pregnant women

Recommendation: FNA should be performed in pregnant women with normal and hypothyroid thyroid nodules. Recommendations: (a) Nodules found early in pregnancy with cytology suggestive of papillary carcinoma should be closely monitored by ultrasound and if they grow significantly by 24 weeks of gestation (defined as a 20% increase in nodule diameter and an increase of at least 2 mm in two or more directions), surgery should be considered immediately. However, if there is no significant change by mid-pregnancy or in patients with thyroid cancer diagnosed in mid-pregnancy, surgery may be delayed until after delivery.  (b) Levothyroxine therapy to maintain TSH at 0.1-1 mU/L can be considered for pregnant women with suspicious FNA results or confirmed PTC. From this recommendation, it is controversial whether surgery should be performed after delivery or in mid-pregnancy when thyroid cancer is detected. This needs to be decided based on the follow-up observations. It is possible to take eugenol suppressive therapy first, which can also be effective. However, in any case, it is not recommended to go for abortion first, and unfortunately a little life is eventually terminated before it comes into the world.
