Introduction to acne

  Concept
  Acne is a common skin disease that causes inflammation of the sebaceous glands or hair follicles on the human skin, and localized lesions produce prickly papules that can squeeze out white or milky white rice-like powder. According to Chinese medicine, there are three types of acne: lung meridian wind-heat type, phlegm-heat type, and punch-anywhere type, each with different symptoms. If the pimples are small and red, they are usually of the lung-heat type; if the pimples are red and swollen and have pus heads, they are mostly of the phlegm-heat type; if the pimples appear or worsen around the time of a woman’s menstrual cycle, they are of the punch-perfect type.” In addition, Luo Guangpu introduced, these three types of acne, each good season. In the summer, phlegm-heat pimples are more common; in the autumn and winter, they are mostly lung meridian wind-heat type pimples; and the unregulated type has no obvious season, but is only related to women’s menstruation.
  Etiology]
  The onset of acne is mainly related to androgens and increased sebum, hyperkeratosis of the hair follicle sebaceous glands, Propionibacterium acnes infection and secondary inflammation, and in some cases, genetics, immunity, endocrine disorders, emotions and diet.
  After puberty, the production of androgens increases or the levels of androgens and estrogens become imbalanced. Increased androgen levels can increase the size of sebaceous glands and increase sebum production. These bacteria (especially Propionibacterium acnes) can hydrolyze the triglycerides in sebum and produce free fatty acids that proliferate and hyperkeratinate at the openings of the sebaceous glands of the hair follicles, which obstruct the sebum secretion channels and cause poor excretion. When sebum and keratin clumps accumulate at the mouth of the hair follicle, acne is formed. In addition, some low molecular peptides produced by Propionibacterium acnes can chemotactic neutrophils, and the hydrolytic enzymes produced by the latter can also cause damage to the hair follicle wall and rupture.
  Diagnostic criteria
  It occurs mostly in young men and women aged 15-30, with lesions mainly on the face and chest and back. The initial basic damage is acne, with no obvious inflammation. The acne is a small nodule with a pinhead size, and the “blackheads” are located at the tip of the open follicle opening and can be squeezed out as hard fat plugs. Small, palpable nodules without “blackheads” are called “whiteheads” or “closed acne”. As the disease progresses, various inflammatory lesions and sequelae may develop, including red papules, pustules, inflammatory nodules, cysts, abscesses, sinus tracts, and scarring. Patients often have multiple rashes present at the same time, but one may predominate. The disease has no obvious self-reported symptoms. Heavy inflammatory lesions can cause discomfort, pain and tenderness, which can be diagnosed based on features such as obstructing grooming.
  【Symptoms
  The hair follicles secrete excessive oil. However, acne is not seborrheic dermatitis. Blackheads and whiteheads: both are non-inflammatory and caused by excessive sebum. The former is black due to the presence of openings, and the surface oil is oxidized by air; the latter has no openings, so the sebum remains white papular knot sequelae: scars (depressions, furrows, chickenpox-like); pigmentation.

  Female patients start to take the medicine after menstrual ovulation, 15 doses until the cessation of menstruation, for 3 consecutive menstrual cycles, male patients take 20d, with a week break, for 3 consecutive cycles as a course of treatment.
