Abstract: Hepatocellular adenoma is a common pathological type of benign tumor of the liver. It is often detected during physical examination and occurs mostly in young women. Sometimes hepatic adenoma and hepatocellular carcinoma cannot be completely distinguished and require surgical resection for pathological confirmation, and hepatocellular adenoma has the potential to transform to hepatocellular carcinoma. Therefore, any tumor of unknown nature found in the liver area is recommended to be surgically removed. For example, in this case, a tumor was found in the liver area during physical examination, and it was confirmed to be a benign tumor after surgical resection, and was cured after resection.
Basic information】Female, 32 years old
Disease Type】Hepatocellular adenoma
Hospital】Liaocheng People’s Hospital
Date of Consultation】November 2021
Treatment plan】Laparoscopic resection
Treatment Period】Inpatient treatment for 1 week, outpatient review after 1 month
Effectiveness】Tumor was completely removed, and the case was confirmed as benign tumor.
I. Initial Consultation
Recently, a young female patient, aged 32, was seen. She was very nervous when she came to the clinic. She complained that she found a tumor on her liver during a physical examination at her workplace, and the nature of the tumor was uncertain. Since the nature of this tumor was not clearly defined, the patient was first advised to have some ancillary tests first, such as intensive abdominal CT or tumor marker tests. If the exact nature could not be determined, surgical resection would be required for pathological diagnosis. The patient expressed his understanding and agreed to be hospitalized.
II. Treatment process
The liver function test was normal, and the viral screening also indicated that there was no viral hepatitis. The tumor markers CEA, CA199 and AFP were all within the normal range, and the intensive abdominal CT examination indicated that the right liver was rich in blood supply tumor, but it was not a typical malignant tumor. Through comprehensive analysis, we believe that the patient’s tumor is more benign, but the specific nature of the tumor cannot be determined, and only surgical resection of the tumor for pathological diagnosis can clarify the diagnosis. Therefore, we communicated with the patient and his family about his current condition, and the patient and his family agreed to perform the surgery. The patient then underwent laparoscopic resection under general anesthesia and the liver tumor was removed. The postoperative pathology confirmed that it was a benign hepatocellular adenoma.
III. Treatment effect
In terms of surgical outcome, the patient recovered exceptionally well after surgery because the tumor was promptly and completely removed and confirmed to be a benign hepatocellular adenoma with no malignant component, and the patient had no other uncomfortable symptoms after resection. The patient was discharged at the end of 1 week of hospitalization and was asked to come to the hospital for regular follow-up after 1 month.
IV. Notes
We are glad that the patient underwent surgery in time to remove the tumor and confirmed that it was benign in nature, eliminating the possibility of the tumor developing into a malignant one. It is recommended that the patient should rest and avoid strenuous labor after discharge. The diet should be based on light and well-digested food, but also gradually increase nutrition, such as gradually increasing the intake of meat, eggs and milk, and then eating more fresh fruits and vegetables. Regularly review liver function or abdominal CT at the outpatient clinic 1 month after surgery to observe whether there are any abnormalities in order to prevent this benign tumor from growing again.
V. Personal insight
This patient was lucky because the tumor was benign and clinically curable through timely surgical resection. Since the incidence of liver cancer is increasing and is correlated with viral hepatitis, alcoholic cirrhosis, and environmental factors, it is recommended that we should have annual routine checkups to help our health. If some uncomfortable symptoms appear in your body, you should not ignore them and seek medical attention as soon as possible so as to achieve timely detection and treatment.
