Diet and treatment of gout patients

  Primary gout cannot be cured because there is no treatment for its cause. Just like diabetes and hypertension, which are more commonly known, gout cannot be cured and requires lifelong treatment.  The purpose of gout treatment is twofold: First, to control the acute attack of gouty arthritis in time.  Second, long-term treatment of hyperuricemia to prevent joint destruction and kidney damage caused by urate deposition.  For this disease that cannot be cured, can we achieve the same quality of life as normal people through simple dietary control and medication? The answer is yes.  1. To what extent should the diet be controlled to be considered appropriate? Gout and hyperuricemia are common abnormal purine metabolism diseases in middle-aged and elderly people. However, in Europe and the United States, strict dietary control is gradually being abandoned, as studies have found that strict dietary control can only reduce blood uric acid values by 1-2 mg/dl. In addition, the widely used table of purine content of food is mostly taken from uncooked food, therefore, it is not known whether there is a need for gout patients to avoid this.  2. An uncontrolled diet can cause the blood uric acid concentration to rapidly reach a state where it can attack at any time. From another point of view, gout patients often have high blood lipids, high blood sugar or high blood pressure and other diseases, which require food control, so dietary control is necessary. According to the actual analysis, the purine content of bean food is not high, and the study of vegetarians such as monks did not find a higher incidence of gout, so the so-called gout patients can not consume bean products is not a scientific statement.  3, the general principle of diet is to avoid eating a high purine diet, such as animal offal, sardines, crab, and other purine-rich foods. Those with medium purine content include fish, shrimp, meat and peas. As for fruits, vegetables, milk, eggs, etc., the purine content is very small.  4.For gout patients, it is very important to remember “three low and one high”: (1) low purine diet; (2) weight reduction; (3) low salt and low fat diet; (4) drink a lot of water.  5. The diet of gout patients should be controlled at about 80-90% of the normal diet, and not fasting and excessive hunger. Carbohydrates can promote the discharge of uric acid, can be moderate consumption of carbohydrate-rich rice, steamed buns, pasta, etc.. Milk, cheese, skim milk powder, egg protein, they are not only high quality protein rich in essential amino acids, but also contain very little purine, which will not have adverse effects on gout patients, so these foods can be the main source of protein for gout patients. The diet should also be light, and salt intake should be limited to no more than 3 grams per day. Fifty percent of the purines in food can be dissolved in soup, so meat and fish should be cooked first and the soup should be discarded before cooking. Also pay attention to the diet in moderation, not overeating should be strictly abstain from alcohol, especially beer, to prevent acute attacks.  6, in order to promote uric acid excretion, to prevent the formation and deposition of uric acid salts, it is advisable to drink more water, so that the volume of urine in more than 2000ml per day. In order to prevent urine concentration, patients can drink a small amount of water before bedtime or in the middle of the night.  7, obese people should reduce calorie intake to reduce body weight. It is worth noting that most of the gout patients are found to be clinically overweight, and the chances of obese patients suffering from gout are also greater than those of the general public, so it is quite important to maintain the ideal weight. However, when gout patients lose weight, they must pay attention to the gradual way, never too fast, because the rapid way to lose weight, but will make the uric acid concentration increase, leading to the deterioration of the disease.  8, avoid overwork, tension, cold and wet, wear comfortable shoes, do not make joint damage, etc. is also an important precautions to prevent gouty arthritis attacks.  9.What kind of drugs can we choose for treatment and prevention? Gouty arthritis is also an inflammatory disease, but unlike pneumonia and cholecystitis, which we usually mention, it is not caused by bacterial invasion and therefore does not require the use of antibiotics. During an acute attack of arthritis, bed rest and elevation of the affected limb should be used until the pain is relieved for 72 hours before resuming activities. Early treatment can lead to rapid relief of symptoms.  10. Commonly used drugs for pain control include colchicine, NSAIDs anti-inflammatory drugs, and hormonal drugs. Drugs commonly used to lower uric acid include uric acid-removing drugs such as benzbromarone and drugs that inhibit uric acid production such as allopurinol. In recent years, a variety of new drugs have achieved good efficacy in preliminary clinical trials, but large-scale trials are still needed before they can be marketed, bringing new hope for gout patients.
