Minimally invasive treatment of kidney stones

  Kidney stone is a high incidence species in urology. The surgical treatment of kidney stones is divided into extracorporeal shock wave lithotripsy, intracorporeal lithotripsy, and open surgery for stone extraction. Currently, there is a trend towards intracorporeal lithotripsy. Intracavitary lithotripsy is divided into two types: transurethral lithotripsy (flexible mirror) and percutaneous nephrostomy lithotripsy, especially percutaneous nephrostomy lithotripsy is the most effective. The standard percutaneous nephrostomy is performed with a standard nephroscope with three generations of ultrasound ballast, and the percutaneous nephrostomy is performed with a small diameter nephroscope or ureteroscope with holmium laser or pneumatic ballast. Both methods have their own advantages and disadvantages. Standard percutaneous nephrolithotomy is commonly used abroad, and both methods are chosen in China.  Both PCNL and MPCNL have their advantages and disadvantages: 1. PCNL has a large channel, and with the third-generation ultrasonic ballistic lithotripsy system, it has a good lithotripsy effect and low pelvic pressure, but it is relatively more traumatic, has more intraoperative and postoperative bleeding, and has expensive equipment.  2.MPCNL has a small channel with little damage, and the lithotripsy effect is good with the high-power holmium laser lithotripsy (or ballistic lithotripsy), but it is easy to cause the pressure in the kidney to rise, and there is a possibility of sepsis and septicemia and other complications caused by reflux; the lithotripsy rate is low for those who are not skilled in surgery.  3. Finding a minimally invasive procedure that can improve the efficiency of lithotripsy, reduce the pressure in the renal union system, and reduce intraoperative and postoperative complications is a difficult problem to be solved in percutaneous renal surgery.  Our percutaneous nephrostomy with holmium laser lithotripsy is a self-designed percutaneous nephrostomy lithotripsy sheath (metal sheath), which can combine the advantages of PCNL and MPCNL, avoiding the disadvantages of both, to achieve minimally invasive, improve the efficiency of lithotripsy, shorten the operation time, reduce the pressure in the renal pelvis, reduce intraoperative bleeding, stone residue, sepsis, septicemia and other complications. It also reduces the occurrence of complications such as intraoperative bleeding, stone residue, sepsis, septicemia, etc., reduces the patient’s pain, reduces the patient’s medical costs, reduces the doctor’s labor intensity, etc., and achieves advanced results in China.
