Ms. Liu is 29 years old, a woman who married at 20, had a child at 21, and had two abortions. 10 days ago, when she was five months pregnant, she was diagnosed with a grape fetus at the city hospital and underwent a negative pressure suction to clear the uterus. She still vividly remembers the scene when the doctor presented her with nearly 1,000 ml of grape-shaped fleshy tissue sucked out of her uterus and she almost fainted. The surgeon, who is well versed in the art of medicine, gave her an intravenous dose of Valium before the operation and a drip of uterine constrictor at the beginning of the operation, and the whole operation started and ended unnoticed. The condition was stable and the mental condition was good all the time. The pathological diagnosis was complete gravidity. One week later, the second operation was performed and no tissue was cleared. According to the doctor’s orders, she was discharged with some oral anti-inflammatory drugs and motherwort cream, and then she had regular follow-up visits to the hospital, and the doctor also specifically advised that she must use condoms for two years for strict contraception. After discharge, Ms. Liu at home to recuperate, but the lingering doubts often haunt her, making her sullen, depressed. Ms. Liu has not been able to figure out why the bad luck happened to her? …… husband is often out drinking and boozing, could it be that the root cause is in him? The reason for this is that the woman’s condition is so bad that she has to go to the clinic. The exact cause of staph is not yet known. However, no matter who is the alcoholic couple, it usually does not cause staph. However, medical research shows that the sexual function of alcoholics decreases, and sperm defects and dead sperm account for 70% of the cases, so the incidence of infertility, miscarriage and stillbirth is higher in alcoholics. In addition, babies born to alcoholic pregnant women often have facial and heart defects, multiple fingers (toes) and other deformities. Fetuses conceived in the same room after alcohol consumption by the couple will have small head malformations, small eye clefts, facial hypoplasia, mental retardation, and congenital stupidity. Epidemiological surveys show that there are significant geographical differences in the incidence of staph, with higher rates in Southeast Asian countries and lowest rates in Europe and the United States. After having had one staph, the recurrence of cases reaches 1% to 3%, which is 40 times higher than the chance of first occurrence of staph. In clinical practice, the incidence of complete staph is much higher than partial staph. The so-called complete gravida means that the uterus is enlarged and the entire uterine cavity is filled with grape-like blister-like tissue, with all placental villi involved and no fetal morphology and structure; partial gravida means that only some of the placental villi are blister-like, and the fetus does not survive to full term, and those who do survive occasionally have complications such as fetal growth retardation and multiple congenital malformations. Etiological studies have shown that the occurrence of hyperemesis is related to the patient’s age and nutritional status. Age older than 40 years or younger than 20 years is a high risk factor for the development of complete staphyloma, while partial staphyloma has no significant correlation with age. Cytogenetic and pathological studies have demonstrated that the chromosomal genome, the genetic material of complete gravida, is of paternal origin, i.e. the egg develops in the absence of the oogenital nucleus (i.e. an “empty egg”, an egg without genetic material) or in the absence of an inactivated oogenital nucleus, and then unites with the spermatogonial nucleus. The majority (about 90%) of these eggs are fertilized by single spermatozoa, and a few are fertilized by double spermatozoa. The karyotype of partial gravida is often triploid, from a normal egg and double sperm, which brings a set of extra paternal chromosomal components; it may also be the result of a normal haploid egg or sperm combining with a diploid gamete (referring to sperm or egg) after a failed decelerated division. After this, it dawned on Ms. Liu that her pregnancy was caused by an abnormal fertilization process. Professor Zhang finally stressed that although it is not the husband’s fault to have a staph, it is better to stop smoking and drinking for the sake of eugenics. Once pregnant, women of appropriate age should maintain good health, be happy and emotionally stable, avoid various diseases and mental stimulation, and go to regular hospitals for pregnancy health checks. For women who are pregnant with a gravida, they should be followed up for at least two years, and only when they are cured or free of malignant changes should they follow medical advice and consider conceiving again.
